Responding To Microaggressions In The Classroom

The term “microaggression” was coined in 1970 to name relatively slight, subtle, and often unintentional offenses that cause harm (Pierce, 1970). Since then, a substantial body of research on microaggressions has demonstrated their prevalence and harmful effects (Boysen, 2012; Solorzan, et. al., 2010; Suárez-Orozco, et. al., 2015; Sue, 2010).

Whether an observer, the target, or the unintentional perpetrator of microaggressions, faculty often don’t know how to respond to them in the moment. We may feel frozen (if the observer) or defensive (if the target or perpetrator). How we respond can shift the communication climate from supportive to defensive, which can have an adverse effect on student learning and comfort (Dallimore, et al, 2005; Souza, et al, 2010).

Despite the feelings of paralysis or reactivity that tend to emerge in response to microaggressions in the classroom, certain practices can be implemented to increase the likelihood of maintaining a supportive climate. The following communication framework is offered as one of many possible response strategies to help faculty feel better equipped to effectively respond when a microaggression occurs.

I developed this framework (first introduced in Chueng, Ganote, & Souza, 2016) as an interactive response one could take to a microaggression by a student in the classroom. The acronym and steps below provide a guide on how to take ACTION rather than feeling frozen when faced with a microaggression.

Ask clarifying questions to assist with understanding intentions.

“I want to make sure that I understand what you were saying.  Were you saying that…?”

Come from curiosity not judgment.

  • Listen actively and openly to their response.
  • If they disagree with your paraphrase and clarify a different meaning, you could end the conversation. If you suspect they are trying to “cover their tracks,” you may consider making a statement about the initial comment to encourage learning.

I’m glad to hear I misunderstood you, because, as you know, such comments can be…”

  • If they agree with your paraphrase, explore their intent behind making the comment.

Can you tell me what you were you hoping to communicate with that comment?”

“Can you please help me understand what you meant by that?

Tell what you observed as problematic in a factual manner.

“I noticed that . . .”

Impact exploration: ask for, and/or state, the potential impact of such a statement or action on others.

“What do you think people think when they hear that type of comment?”

“As you know, everything speaks. What message do you think such a comment sends?”

“What impact do you think that comment could have on …”

Own your own thoughts and feelings around the impact.

“When I hear your comment I think/feel…”

“Many people might take that comment to mean…”

“In my experience, that comment can perpetuate negative stereotypes and assumptions about… I would like to think that is not your intent.”

Next steps: Request appropriate action be taken.

“Our class is a learning community, and such comments make it difficult for us to focus on learning because people feel offended. So I am going to ask you to refrain from stating your thoughts in that manner in the future. Can you do that please?”

“I encourage you to revisit your view on X as we discuss these issues more in class.”

“I’d appreciate it if you’d consider using a different term because it is inconsistent with our course agreement regarding X…”

When practiced, the ACTION framework can be a tool that is quickly retrieved out of your mental toolbox to organize your thoughts and unpack the microaggression in a way that addresses the situation and cools down tension.

When students make comments that are microaggressive in the classroom, doing nothing is a damaging option (Souza, Vizenor, Sherlip, & Raser, 2016). Instead, we can engage thoughtfully and purposively in strategies that maintain a positive climate that is conducive to learning and models the skills needed in responding to microaggressions in any context (Souza, 2016).

What Are The Effects of Total Isolation? An Expert Explains

Imagine being confined to a small, dark room, with no social interaction whatsoever for 30 days. Not many people would jump at this opportunity. But, in November 2018, a professional US poker player Rich Alati bet US$100,000 that he could survive 30 days alone and in total darkness. He was kept in a small, completely dark room with nothing but a bed, fridge and bathroom. Even with all the resources he needed to survive, Alati couldn’t last the month. After 20 days he negotiated his release, taking a payout of US$62,400.

There are countless negative effects that social isolation and extreme isolation can have on our minds and bodies. Alati was no exception, reporting that he experienced a range of side effects, including changes to his sleep cycle, and hallucinations. But why is isolation so difficult for humans to withstand?

One of the reasons that living in isolation is difficult is because humans are social creatures. Many people that have lived in isolated environments – such as researchers stationed in Antarctica – report that loneliness can be the most difficult part of the job. Yossi Ghinsberg, an Israeli adventurer and author who survived weeks alone in the Amazon, said that loneliness was what he suffered from most and that he had created imaginary friends to keep himself company.

Loneliness can be damaging to both our mental and physical health. Socially isolated people are less able to deal with stressful situations. They’re also more likely to feel depressed and may have problems processing information. This in turn can lead to difficulties with decision-making and memory storage and recall.

People who are lonely are also more susceptible to illness. Researchers found that a lonely person’s immune system responds differently to fighting viruses, making them more likely to develop an illness.The impacts of social isolation become worse when people are placed in physically isolating environments.

For example, solitary confinement can have negative psychological effects on prisoners – including significant increases in anxiety and panic attacks, increased levels of paranoia, and being less able to think clearly. Many prisoners also report long-term mental health problems after being held in isolation.

Natascha Kampusch – an Australian woman who was kidnapped at the age of ten and held captive in a cellar for eight years – noted in her biography that the lack of light and human contact mentally weakened her. She also reported that endless hours and days spent completely isolated made her susceptible to her captor’s orders and manipulations.

Alone in the dark

The effects of isolation can become even more pronounced if you experience it in total darkness, causing both physical and psychological consequences. One impact of being in complete darkness is that it can wreck your sleep cycle. Two of the key mechanisms for sleep cycle regulation, the hormone melatonin and the brain’s suprachiasmatic nucleus, both rely on light to function.

Daylight reduces our levels of melatonin, helping us feel awake.

Daylight also helps the suprachiastmatic nucleus to reset our waking time if our sleep cycles start to drift. Without daylight, our 24-hour circadian rhythm can change. This explains why people exploring cave systems, for example, may find that their sleep-wake cycle becomes disrupted. This means that the time they feel like going to sleep doesn’t stay in a regular pattern and can shift each day.

Disruptions to our circadian rhythm can also make us feel depressed and fatigued. This has also been linked to increased cancer risk, insulin resistance and heart disease, as well as other physical problems such as obesity and premature ageing.

People placed in isolation may also experience hallucinations. The lack of stimuli causes people to misattribute internal thoughts and feelings as occurring in the outer environment. Essentially, hallucinations happen because of a lack of brain stimulation.

In fact, Alati revealed he began experiencing hallucinations by his third day in isolation, ranging from seeing the room fill up with bubbles, to imagining that the ceiling had opened up to show him a starry sky. People in total isolation may also feel that there is a ghostly presence or someone watching them.

While the impact of total isolation can be severe, the good news is that these effects are reversible.

Exposure to daylight can normally correct sleep-wake patterns – though this might take weeks, or even months in some cases, before it’s fully adjusted. Reconnecting with other humans can reduce loneliness and help restore us to good mental and physical health. However, some people who have been held in social isolation against their will may develop long-term mental health conditions, such as post-traumatic stress disorder (PTSD).

But some people who have faced the challenge of being alone for an extended period of time may show personal growth – including emotional growth, feeling closer to family and friends, and having a better perspective on life – as a result of their experience. After 20 days willingly spent in total isolation, even Alati said he’s changed – reporting that the experience gave him a greater appreciation for people and life, better attention and focus, and overall feeling happier than before.

PFAS Forever Chemicals Linked To Liver Cancer In First of Its Kind Study

New research has reported a link between elevated blood levels of perfluooctane sulfate (PFOS) and increased risk of liver cancer. The study is the first to directly associate exposure from this “forever chemical” to liver cancer in humans.There are thousands of different chemicals under the umbrella of per- and polyfluoroalkyl substances, or PFAS. These substances are often informally referred to as “forever chemicals” because they can persist in the environment for decades.

For much of the 20th century PFAS chemicals were used in a broad variety of manufacturing contexts. However, over the past 20 years an increasing body of evidence has revealed exposure to PFAS can contribute to a number of deleterious health effects, from low birth weight to hypothyroidism.

Following on from animal and lab-based studies suggesting PFAS exposure is potentially damaging to liver cells, this new research offers the first robust investigation into the relationship between the chemicals and liver cancer in humans.

The power of this new research is in its leveraging of data from a multi-decade health study following more than 200,000 people since the early 1990s. This allowed the researchers to look at historic blood levels of PFAS in subjects years before they presented with cancer.“Part of the reason there has been few human studies is because you need the right samples,” explained Veronica Wendy Setiawan, an author on the new study. “When you are looking at an environmental exposure, you need samples from well before a diagnosis because it takes time for cancer to develop.”

Several types of PFAS were found in blood samples from subjects who ultimately went on to develop liver cancer. But one particular chemical stood out, perfluooctane sulfate (also known as PFOS). Those subjects with the highest blood levels of PFOS were 4.5 times more likely to develop liver cancer compared to the group with the lowest PFOS blood levels.

The study also homed in on some potential mechanisms by which PFOS exposure could be causing liver cancer. Four particular metabolites were found to link PFOS levels with liver cancer: “glucose, butyric acid (a short chain fatty acid), α-Ketoisovaleric acid (a branched-chain α-keto acid), and 7α-Hydroxy-3-oxo-4-cholestenoate (a bile acid).”

“We believe our work is providing important insights into the long-term health effects that these chemicals have on human health, especially with respect to how they can damage normal liver function,” explained lead researcher on the project Leda Chatzi. “This study fills an important gap in our understanding of the true consequences of exposure to these chemicals.”

The new findings come just days after another study found levels of PFAS in rainwater often exceed new safety limits set by the Environmental Protection Authority (EPA). That research called for a global phase-out of PFAS from all manufacturing contexts as soon as possible. An expert committee formed by the United Nations to review persistent pollutants in 2019 also called for the elimination of all use of PFAS chemicals to, “protect human health and the environment from its harmful impacts.”

A New Generation of Weight Loss Drugs Makes Bold Promises, But Who Really Wins

In the months after having her second child, Sarah found herself fed up. The 40-year-old Seattle resident was cutting carbs and sugar, and exercising regularly, but couldn’t seem to shed the pounds she had put on during pregnancy. So when an email newsletter mentioned a new weight-loss drug called Wegovy, Sarah decided to give it a try. Eight months later, she is out more than $10,000—and down more than 60 lbs.

“Wegovy made losing weight almost effortless,” Sarah, whose name has been changed to protect her identity, told Quartz. “I’m not hungry often anymore and it doesn’t take any willpower to eat less. I simply don’t have any desire to overeat.”

Sarah is one of 125,000 US-based patients now taking Wegovy (whose generic name is semaglutide), a member of a new class of weight-loss drugs. These drugs work differently than the appetite suppressants popular among previous generations of dieters. They are also hitting the market at a different moment: one in which people are more eager than ever for realistic, science-based methods for addressing excess weight, even as a growing faction of activists and doctors voice skepticism of weight as an accurate measure of health.

A new class of weight-loss drug

In the mid-1990s, experiments on Gila monster venom found it contained hormones that could help lower blood sugar. That led to the diabetes drug Ozempic, which ultimately went on the market in 2018. People on that drug discovered a funny side effect: They lost weight.

In 2021, that same compound was approved by the US Food and Drug Administration (FDA) under the name Wegovy for the express purpose of weight loss. Drugs like Wegovy work in more complex ways than simply suppressing appetite, and promise fewer (though not zero) side effects.

Like Wegovy, many of these drugs were originally approved for other conditions; liraglutide (brand name Saxenda for weight loss) was also originally approved as a diabetes drug (Victoza). In fact, semaglutide and liraglutide work similarly in the body: They’re known as GLP-1 receptor agonists because they activate receptors for the glucagon-like peptide-1 (GLP-1) hormone, reducing appetite by slowing digestion and the rate at which the body takes up glucose.

Perhaps most important, the new drug promise significant weight loss. “The previous weight loss drugs were just modestly effective,” says John Buse, an endocrinologist at the University of North Carolina School of Medicine. The average patient would lose 5% of their body weight, in some cases up to 8%. But with semaglutide, he says, “we’ve gotten the kind of weight loss that makes people pay attention: 10-15% of body weight. That’s the average weight loss—half of people are losing more than that. It’s a gamechanger in the conversation…now that we have medicines for which a substantial proportion of patients can expect to lose 30 to 50 lbs.”

In one 68-week pre-approval clinical trial, patients on Wegovy did indeed lose 14.9% of their body weight on average, compared with 2.4% for people on a placebo. (Although, as several writers and scholars have pointed out, the study was funded by Novo Nordisk, which makes Wegovy.) Given the average weight of trial participants—100 kg, or 220 lbs.—that meant weight loss of about 15 kg, or 33 lbs. Other drugs in development have had similar results. In a recent trial for one called tirzepatide from Eli Lilly, more than half of patients lost at least 20% of their body weight—50 lbs. in many cases.

What it takes to lose weight

This new class of drugs is entering a market that at first glance seems ripe for breakthrough. According to the US Centers for Disease Control and Prevention (CDC), 42% of Americans—70 million people—meet the criteria for obesity (having a BMI of 30 or more). At one point or another, most of those people will try a diet and exercise regimen to lose weight.

But a growing body of research shows that diets are not an effective way to lose weight and keep it off. “Obesity is a complex disease… ​for most people, lifestyle modifications, diet, and exercise are just not enough,” says Katherine Saunders, a doctor at the Comprehensive Weight Control Center at Weill Cornell Medicine and co-founder of Intellihealth, an app-based platform that brings evidence-based obesity treatment to patients.

In part because of that complexity, bariatric surgery has since 2009 been considered the standard of care for patients looking to lose a substantial amount of weight. But these procedures can be invasive and expensive, and can come with significant and long-lasting complications.

The dearth of other options leaves some patients and doctors excited about this new generation of drugs. “Right now, the field is really looking for more efficacy, number one. People will do almost anything to lose weight,” says Buse. “We have more than just surgery now for promoting substantial weight loss. The most exciting thing is that obesity is on the ropes.”

A complicated picture

While hopes are high, the realities of taking these drugs can be more complicated for patients. There are often side effects—the most common for semaglutide and liraglutide are diarrhea, vomiting, and nausea. On Wegovy, Sarah says she’s experienced diarrhea so severe that a few times she had to delay her next dose.

Physicians can sometimes gloss over or downplay those effects. But a visit to dedicated Reddit pages for these drugs shows whole communities of patients struggling to adhere to the regimen when they’re feeling sick, and seeking support from a community to understand whether what seems like a severe reaction is normal. (Novo Nordisk did not respond to a request for comment.)

How well a patient can tolerate a drug “is something we think about quite a lot,” Saunders says. “We always start with lower doses and increase gradually as tolerated. Everyone is different. We keep in close touch with the patient and monitor them closely.”

And while these new drugs are relatively well-studied, there are still unknowns. They seem to help patients keep weight off more reliably than diet and exercise alone, but those benefits fade after people stop taking the drugs, and patients do often regain weight. There are also questions about long-term effects. In 1997, weight loss drug fenfluramine/phentermine (fen-phen) was pulled off the market after it was found to cause heart problems. More recently, Belviq (lorcaserin), which the FDA approved for weight loss in 2012, was pulled from the US market in 2020 because long-term use was found to increase the incidence of various types of cancers.

Even if a patient does want to go on one of these drugs, she might not be able to. Many patients keen to try Wegovy can’t access it at the moment, due to a supply chain issue that its manufacturer doesn’t expect to resolve until later this year. Even then, most US health insurers, including Medicare, do not cover drugs like Wegovy, and paying out of pocket can cost thousands of dollars per month. After Sarah’s doctor told her she doesn’t prescribe Wegovy, Sarah secured a prescription through an online health provider; she pays for it out of pocket.

The lack of insurance coverage is in spite of the fact that the American Medical Association declared obesity to be a disease in 2013. “The conversation around insurance coverage needs to be had with insurance companies, but also with employers,” says Kimberly Gudzune, the medical director for the American Board of Obesity Medicine. “It needs to be seen as an investment in your workforce.” The Treat and Reduce Obesity Act, which would expand Medicare to include obesity treatments, has been introduced to US Congress every year since 2012, but has never passed.

America’s love/hate relationship with weight

Though excess body fat was once considered a sign of wealth or fertility, over the past century a stigma has developed against larger bodies. Today doctors associate excess weight with medical conditions like heart disease, cancer, type 2 diabetes, sleep apnea, osteoarthritis, and depression. Studies also show that life is harder when you move through the world in a larger body. Fat people are less likely to be hired for a job, are paid less, are less likely to get married, and are less likely to be happy (though not if they’re living around other fat people). One 2006 study found that 46% of respondents would rather give up one year of life than be obese; 5% said they’d rather lose a limb.

The current state of research makes it impossible to unravel the full complexity of weight and health, but the conversation is starting to accommodate more nuance. Ubiquitous metrics such as body mass index are increasingly understood to be unreliable indicators (though doctors often still use them), and even the language around larger bodies is under review. Many physicians use “obese” to describe people who have excess weight or a BMI over 30, but activists are shying away from the word. “The reason…we are reluctant to use the words ‘overweight’ and ‘obesity’ is that they are made up, they can change,” says Tigress Osborn, a fat activist and chair of the National Association to Advance Fat Acceptance.

In fact, some research suggests that fat may have a protective effect on the body. “The body’s weight-regulating mechanism is about survival. It’s a system with more moving parts than we understand,” says Marilyn Wann, a fat activist and author of the book Fat!So? “Trying to remove weight from an individual or from the population is like trying to take a sledgehammer to the weather—we don’t know the unintended negative consequences we’re going to create.”

There are signs that in the future physicians may be more accepting of bodies of different sizes. But as weight loss drugs get more effective and more available, those cultural gains for body positivity (or body neutrality, or fat acceptance) may also be called into question.

A new relationship between doctors and patients

Overweight patients who come to see Shelly Crane might have an experience they’ve never had before. “I don’t initiate a weight-loss conversation with a patient,” says Crane, a family physician at Advocate Aurora Health in Milwaukee, Wisconsin. Most weight-loss programs come with more risk of harm than good, she says, and there’s not enough evidence that people who do lose weight are healthier in the end.

Crane doesn’t regularly prescribe new drugs for weight loss, though she says more patients are coming in and asking for them lately. Instead, she prefers to keep conversations focused on goals of care. “Patients say, ‘I know I need to lose weight,’ and I say, ‘Why do you think you need to lose weight? What would change in your life if your weight was lower?’” That gives her an opening to talk about health more broadly—how is the patient’s sleep? Their diet? Their mobility? “I try to stay in my sphere of what I’m able to do as a family doctor and really address the root of the health issue as much as I can.”

Crane was drawn to this approach by listening to her patients talk about experiencing size discrimination, and by following the work of fat activists such as Ragen Chastain and Aubrey Gordon. Though she’s been trained in a more integrative style of medicine, her approach toward body acceptance was also shaped by her discovery of intuitive eating during medical school. Since then, she’s been working on deprogramming herself and her colleagues from anti-fat bias.

Crane is part of a burgeoning movement among doctors to improve the treatment of larger patients. For some, that means skipping the dreaded weigh-in, a practice that is somewhat controversial within medicine. Medical organizations like the Association of American Medical Colleges also offer guidelines to reduce anti-fat bias among clinicians.

For doctors, the updated approach at least engenders trust, which can in turn get patients to seek medical care more frequently and improve their overall health. At most, it broadens the definition of what “healthy” means, and looks like.

Some fat activists see this shift as an important step. “The thing we hear most often from the public is, ‘I thought I had this thing, but all the doctor wanted to talk to me about is weight loss, and now the thing is worse,’” Osborn says. “It’s progress to have people in the medical establishment recognizing that there are other healthcare concerns besides weight, if weight is a healthcare concern.”

The hope is that this evolution continues. Activists want more people, in the medical profession and outside of it, to respect their autonomy. That becomes even more pressing in a possible future filled with weight-loss drugs—a future where a person can simply take a drug and stop being fat. “The ease with which I could become smaller—why should I? That should be up to me. Just like, if you believe it’s a medical disorder, the treatment I choose should be up to me,” Osborn says. “Like with anything else, if you believe fat is a disorder, we should let people decide whether people will get treated or not.”

“Fatness isn’t a problem to be solved in and of itself. It is not the root cause of all ills, as much as [medicine] would like to think it is,” Crane says. “We can help people live full, rich lives when we focus on goals of care and not on weight.”

Sadly, we all get lonely from time to time, and social distancing and self-isolation certainly don’t help. Here, a psychologist offers her advice for overcoming these feelings of loneliness.

Have you ever gotten into bed at the end of the day and realized that you haven’t spoken out loud to anyone since the day before? Or simply found yourself feeling completely and utterly alone?

We live in a hyper-connected world, and yet we’re lonelier than ever before, a situation that is certainly not helped by the current UK lockdown. We have more social media followers than real-life friends, and it’s easier to swap digital messages with strangers on the other side of the planet than it is to sit down for a chat with an actual person – especially now that we’re social distancing and putting ourselves in isolation.

Despite being traditionally viewed as an affliction that’s limited to the elderly, it’s now 16-24 year olds who make up the loneliest age group of all, a finding confirmed in the UK’s largest study on the subject, The Loneliness Experiment by BBC Radio 4. The study included over 55,000 people and found that 34% of 25-34 year olds are lonely ‘often or very often’ while 36% of 34-44 year olds felt the same.

Now, scientists are warning of the damaging effects of a ‘loneliness epidemic’, with loneliness even being equated to the health equivalent of smoking 15 cigarettes a day.

As many of us will know, we don’t need to be physically alone to feel lonely. A toxic friendship or relationship can be incredibly isolating, for example, while spending too much time with people we don’t feel close to can have damaging effects on our psyche, even if we’re only interacting with them through our phones. Loneliness affects people in different ways, and for this reason there are four distinct types of loneliness identified by psychologists: emotional, social, situational and chronic.

But how do we know what type of loneliness we’re experiencing and, more importantly, how can we tackle it? Here, Stylist talks to registered psychologist Dr Becky Spelman, and hears from women who have experienced loneliness – and managed to keep it at bay.

Emotional Loneliness

“Those who are emotionally lonely will find it difficult to improve things without tackling the root of the problem,” says Dr Spelman. “Emotional loneliness is not circumstantial but, rather, comes from within.”

Dr Spelman recommends therapy to help tackle the root cause of these feelings of emotional loneliness. “Working with a therapist, possibly with a technique such as behavioural cognitive therapy, or attending group therapy, is likely to lead to the best possible outcome,” she says.

“The person in question can start to understand why they are lonely, how their background and experiences have contributed to behaviours that make things worse, and how they can develop a new, and more useful, set of behaviours.”

Situational Loneliness

Many millennials choose to work abroad for a few years in their 20s and 30s, and the rise in solo travel means a great number of us are planning to jet off on solo adventures once the coronavirus pandemic has passed.

While these plans are undoubtedly exciting, it can also be a period of adjustment as we try to make new friends while simultaneously getting to grips with a new culture and way of life – potentially leading to situational loneliness, says Dr Spelman.

“Situational loneliness can result from being in circumstances that make developing friendships difficult,” she says. “Examples include those living abroad, perhaps in a place where they do not speak the language perfectly, stay-at-home mothers (or fathers) with young children, or those with a physical or intellectual disability that makes it difficult to get out and about.”

Situational loneliness is something that digital writer Susan experienced when she moved to Dubai for two and a half years in 2013.

“When I first moved to the Middle East, I was completely alone,” she says. “I didn’t have a friend or a family member I could call when the going got tough, or even just someone to have a cup of tea with. It was my choice to move there for work. And it was my choice to move there as a single woman. But that doesn’t mean it was easy.

“The first month was particularly difficult as I spent two weeks in a hotel on my own, surrounded by a completely different culture, customs and language. I felt the sting of loneliness as I dined on my own every evening and looked for a flat share – going from one viewing to the next without understanding a word of Arabic. But after I found somewhere to call home, I made friends with my housemates by putting myself out there and never turning down an invitation to do something new. Thankfully, I pushed myself to get to know them and my new country of residence, too.”

But situational loneliness doesn’t just arise in those who relocate alone, as social media editor Sarah discovered when she moved countries with her partner. Sarah left her Sydney home in March 2017 to join her partner in London, where he had arrived a month earlier to start a new job and find the pair a home to live in – although she admits that “didn’t mean it was smooth sailing”.

“On my first night an emotional (jetlagged) me fell asleep whimpering into my partner’s shoulder, ‘I had to give up a lot to be here’. Although very dramatic, I was more right than wrong. I knew making friends as an adult, outside of freshers’ week or a friend-of-a-friend introduction, would be preceded by a stint of loneliness – something I dreaded.

“The reality of that fear set in a week later. Without a job, I had less interaction with people and felt starved of company. I took to being overly chatty to baristas and the person on the till at Sainsbury’s. Each day I would surf LinkedIn and talk about the weather with anyone mildly inclined to respond. I would look forward to my partner coming home after work for the sheer joy of talking to someone who knew me beyond what groceries I was buying that day.”

Sarah did make friends in the city but she is also aware that “having a partner at home slowed my efforts”.

“There would be times I would crave going out with a friend rather than my partner. But the reality was that in those initial weeks he was my only friend in London. And that made me feel tied to him in a way that we never were in our home town of Sydney.

“It did take a considerable amount of effort to form friendships outside of his friendship circle (formed from his middle-school years in south London). Eighteen months on, I keep working on those friendships outside of my relationship – because I now know how very valuable they are.”

Pushing yourself to make new friends – and crucially, maintain those friendships – is exactly what Dr Spelman recommends for someone who is grappling with situational loneliness. And while we can’t currently make an active effort to meet new people in person due to coronavirus, there are alternative solutions.

“The best approach here is a proactive one,” she says. Once we are out of lockdown and able to socialise in person again, she suggests “joining a language class or a hobby group or getting in touch with like-minded people and actively courting friendships, to help overcome loneliness in these circumstances.

“The internet can help. While socialising online is not the same as meeting up with friends for coffee or a drink, establishing a support network online can help to maintain a sense of being liked and wanted, and keep social skills alive.”

Social Loneliness

“Social loneliness is typically experienced by those who have problems in social situations because of shyness, social awkwardness, or a sense of low self-esteem that makes them doubt their capacity to be competent and entertaining in social circumstances,” Dr Spelman explains.

“Different approaches can help. For example, if the root issue is one of low self-esteem, tackling this first should make a positive difference. Trying a structured approach to socialising, such as joining an online or virtual group that gets together to discuss or engage in a particular hobby, can be a good way to start to end a vicious circle.”

Chronic Loneliness

“Chronic loneliness is the term used to describe those who have been lonely for so long that it has become a way of life to them,” explains Dr Spelman. “If solitude has become part of their nature, it can be tricky to break the cycle.”

Chronic loneliness is something that Lyla, now 26, experienced when she moved to London to start university at the age of 18. Lyla had previously lived in Nottingham where she had a solid group of friends who all lived nearby, and spent a lot of time together. Moving to a new city triggered feelings of loneliness for her, which became entrenched over her first two and a half years in the city.

“It was a really bewildering, lonely time,” she says. “The jump of being plonked into a huge arts school, in just one of hundreds of halls of residence in a sprawling city I didn’t understand, made me retreat into myself and I struggled to make friends in the face of it all.

“I spent a lot of time in my tiny room making my mum talk to me for hours and hours on the phone, while I slowly found my footing and met people I connected with. It took me about two and a half years to learn not to hate London, but sticking it out meant it slowly got better, and nine years later nowhere has ever felt more like home.”

Dr Spelman notes that chronic loneliness is often a by-product of circumstance, such as self-isolation, although unlike situational loneliness, it can go on for so long that it almost becomes a way of life.

“Examples include the elderly whose friends have largely passed away or moved into nursing care, while adult children live far away, or those who are inhibited from socializing by a controlling partner or other circumstances that feel out of their control,” she says. “It is important to remember that we all deserve friends and a social life and that there is nothing wrong with asking for help or making the first step.”
