Your Kids are Adorable Germ Vectors. Here’s How often They Get Your Household Sick

I wanted to report this story last month, but I was too sick with COVID. My kid gave it to me. My colleagues on the health reporting team would have tackled the story, but they’ve been sick, too, thanks to their children. (Just last week, one colleague dropped off her daughter for her first day back at preschool after recovering from a bug, only to pick her up that same afternoon, sniffling from a new illness. Yikes.)

And we’re far from alone in our woes. “Like so many parents out there, you know, my husband and I have been sick all winter. We’ve been sneezing, coughing, had fevers. It’s gross,” says Dr. Rachel Pearson, a pediatrician at The University of Texas Health Science Center at San Antonio and University Hospital. She’s also the mother of 2-year-old Sam.

I feel like half the time he has a virus, has a runny nose, is coughing – to the point where my dad was like, ‘Is there something wrong with Sam?’ ” she says. With flu, RSV, colds and COVID all coming at once, it can feel like things may be worse than ever for parents of little kids. But as Pearson tells her dad – and the parents of her own young patients – this seemingly never-ending cycle of sniffles is normal, if miserable.

When I counsel parents, I say you can have a viral infection every month. Some kids are going to cough for four weeks to six weeks after a virus. And so they’re going to catch their next virus before they even stop coughing from the last one.” In fact, if you’ve ever described your child as an adorable little germ vector, you’re not wrong, says Dr. Carrie Byington, a pediatric infectious disease specialist and executive vice president for the University of California Health System. And she’s got hard data to back that up.

“We all think it, but it was really incredible to have the definitive proof of it,” says Byington. The “proof” she’s referring to comes from a study she and her colleagues began back in 2009, when she was at the University of Utah. They wanted to understand the role kids play in the transmission of respiratory viruses in their homes. So they recruited 26 households to take nasal samples of everyone living in the home, every week, for an entire year. What they found was eye-opening.

“We saw as soon as a child entered the house, the proportion of weeks that an adult had an infection increased significantly,” Byington says. And more kids meant more infections. For families with two, three or four kids, someone at home had an infection a little more than half the year. Families with six kids had a viral detection a whopping 87% of the year. Childless households, on the other hand, only had a viral detection 7% of the year.

The findings also suggest that the youngest kids are the ones bringing germs home most often: Children under age 5 were infected with some kind of respiratory virus a full 50% of the year – twice as often as older kids and adults. And while a viral detection didn’t always translate into illness, when they were infected, the littlest kids were 1.5 times more likely to have symptoms, like fever or wheezing.

And that’s just respiratory viruses. As Byington notes, the study wasn’t even looking at other kinds of infections, such as strep throat, which is caused by bacteria. “So obviously, there could be other things that happened throughout the year to even make it seem worse,” she says.

Byington says all of this means that, in the grand scheme of things, it’s normal for kids to be getting all these viruses. But it’s all more intense right now because of the disruptions of the pandemic. Children were kept at home instead of going to daycare or school, where they would typically be exposed to viruses and bacteria one after another, she says.

As children returned to regular routines, “there were lots of kids ages 1, 2 and 3 who had never really seen a lot of viruses or bacteria,” Byinton says. “And so what might have been spread out in the past over 12 months, a year, they were now seeing it all at once in this very concentrated time.”

Byington says the pandemic also disrupted the seasonality of viruses. Flu season hit earlier than usual this year, as RSV and COVID were also circulating. Young children without prior exposure to these viruses were hit especially hard.

Pearson notes that’s because kids are likely to have a more severe course of illness the first time they encounter a virus like RSV, before they have some level of immunity. She says there’s a larger cohort of kids this year that didn’t have that prior exposure.

And there is evidence that younger kids who get multiple infections – say, COVID and RSV– at the same time can end up with more severe illness than if they’d gotten just one virus at a time. The end result is that many pediatric hospitals and care units have seen a surge in sick kids over the fall and winter. That includes University Hospital in San Antonio, where Pearson sees hospitalized kids in the acute care unit.

Nationwide, “pediatric care right now is at this point of strain,” Pearson says, not just because of the current surge but because of an underinvestment that predates the pandemic. And “the kids who get admitted to the hospital are the tip of the iceberg,” Pearson says. For every kid sick enough to be hospitalized, there are likely many more with the same virus recuperating at home, she says.

The good news is that the viral stew seems to be easing up. Recent data from the CDC show the number of emergency department visits for flu, COVID and RSV dropped to the lowest they’ve been since September for all age groups. But of course, the respiratory virus season isn’t over yet.

As for families who are currently living in what one headline memorably dubbed “virus hell,” Byington hopes the findings of the BIG-LoVE study should offer some comfort that eventually this, too, shall pass. “It’s nice to have done the study and to offer some real-world data to families that what they’re living through is normal and will pass and their children will be well,” she says.

Scientists Finally Know Why People Get More Colds and Flu In Winter

A chill is in the air, and you all know what that means — it’s time for cold and flu season, when it seems everyone you know is suddenly sneezing, sniffling or worse. It’s almost as if those pesky cold and flu germs whirl in with the first blast of winter weather.

As respiratory viruses strain US health care systems, Biden administration tells states how it’s ready to help. Yet germs are present year-round — just think back to your last summer cold. So why do people get more colds, flu and now Covid-19 when it’s chilly outside?

In what researchers are calling a scientific breakthrough, scientists behind a new study may have found the biological reason we get more respiratory illnesses in winter. It turns out the cold air itself damages the immune response occurring in the nose.

“This is the first time that we have a biologic, molecular explanation regarding one factor of our innate immune response that appears to be limited by colder temperatures,” said rhinologist Dr. Zara Patel, a professor of otolaryngology and head and neck surgery at Stanford University School of Medicine in California. She was not involved in the new study.

In fact, reducing the temperature inside the nose by as little as 9 degrees Fahrenheit (5 degrees Celsius) kills nearly 50% of the billions of virus and bacteria-fighting cells in the nostrils, according to the study published Tuesday in The Journal of Allergy and Clinical Immunology.

“Cold air is associated with increased viral infection because you’ve essentially lost half of your immunity just by that small drop in temperature,” said rhinologist Dr. Benjamin Bleier, director of otolaryngology translational research at Massachusetts Eye and Ear and an associate professor at Harvard Medical School in Boston.

“it’s important to remember that these are in vitro studies, meaning that although it is using human tissue in the lab to study this immune response, it is not a study being carried out inside someone’s actual nose,” Patel said in an email. “Often the findings of in vitro studies are confirmed in vivo, but not always.”

A hornet’s nest

To understand why this occurs, Bleier and his team and coauthor Mansoor Amiji, who chairs the department of pharmaceutical sciences at Northeastern University in Boston, went on a scientific detective hunt.

Here’s how to know when your child is too sick for school. A respiratory virus or bacteria invades the nose, the main point of entry into the body. Immediately, the front of the nose detects the germ, well before the back of the nose is aware of the intruder, the team discovered.

At that point, cells lining the nose immediately begin creating billions of simple copies of themselves called extracellular vesicles, or EV’s.“EV’s can’t divide like cells can, but they are like little mini versions of cells specifically designed to go and kill these viruses,” Bleier said. “EV’s act as decoys, so now when you inhale a virus, the virus sticks to these decoys instead of sticking to the cells.”

Those “Mini Me’s” are then expelled by the cells into nasal mucus (yes, snot), where they stop invading germs before they can get to their destinations and multiply. “This is one of, if not the only part of the immune system that leaves your body to go fight the bacteria and viruses before they actually get into your body,” Bleier said.

Once created and dispersed out into nasal secretions, the billions of EV’s then start to swarm the marauding germs, Bleier said.

“It’s like if you kick a hornet’s nest, what happens? You might see a few hornets flying around, but when you kick it, all of them all fly out of the nest to attack before that animal can get into the nest itself,” he said. “That’s the way the body mops up these inhaled viruses so they can never get into the cell in the first place.”

A big increase in immune power

When under attack, the nose increases production of extracellular vesicles by 160%, the study found. There were additional differences: EV’s had many more receptors on their surface than original cells, thus boosting the virus-stopping ability of the billions of extracellular vesicles in the nose.

“Just imagine receptors as little arms that are sticking out, trying to grab on to the viral particles as you breathe them in,” Bleier said. “And we found each vesicle has up to 20 times more receptors on the surface, making them super sticky.”

Cells in the body also contain a viral killer called micro RNA, which attack invading germs. Yet EVs in the nose contained 13 times micro RNA sequences than normal cells, the study found. So the nose comes to battle armed with some extra superpowers. But what happens to those advantages when cold weather hits?

To find out, Bleier and his team exposed four study participants to 15 minutes of 40-degree-Fahrenheit (4.4-degree-Celsius) temperatures, and then measured conditions inside their nasal cavities.

“What we found is that when you’re exposed to cold air, the temperature in your nose can drop by as much as 9 degrees Fahrenheit. And that’s enough to essentially knock out all three of those immune advantages that the nose has,” Bleier said.

It seems like everyone’s getting sick this winter. Parents and health care workers, how are you coping? In fact, that little bit of coldness in the tip of the nose was enough to take nearly 42% of the extracellular vesicles out of the fight, Bleier said.

“Similarly, you have almost half the amount of those killer micro RNA’s inside each vesicle, and you can have up to a 70% drop in the number of receptors on each vesicle, making them much less sticky,” he said.

What does that do to your ability to fight off colds, flu and Covid-19? It cuts your immune system’s ability to fight off respiratory infections by half, Bleier said.

You don’t have to wear a nose sock

As it turns out, the pandemic gave us exactly what we need to help fight off chilly air and keep our immunity high, Bleier said. Wearing a mask can protect you from cold air that can reduce your immunity, an expert says.

“Not only do masks prrhinologist Dr. Benjamin Bleierotect you from the direct inhalation of viruses, but it’s also like wearing a sweater on your nose,” he said. Patel agreed: “The warmer you can keep the intranasal environment, the better this innate immune defense mechanism will be able to work. Maybe yet another reason to wear masks!”

In the future, Bleier expects to see the development of topical nasal medications that build upon this scientific revelation. These new pharmaceuticals will “essentially fool the nose into thinking it has just seen a virus,” he said. “By having that exposure, you’ll have all these extra hornets flying around in your mucous protecting you,” he added.

How to Hire With a Vaccine Mandate in Place

Asking prospective employees about their vaccination status can be awkward–and possibly lead to legally sticky situations. Some prospective employees will appreciate it if your company is mandating Covid-19 vaccinations. While surgical-grade facemasks have proven somewhat effective at preventing the spread of Covid-19, a vaccine is currently the only true way to protect yourself and others from getting sick.

Others are less convinced. And if your company is in a hiring bind, as many are these days, you might need to codify and justify your safety policies. Here’s a primer on how to hire with a vaccine mandate in place.

Be transparent.

Nicolas Holand–founder of GooseSmurfs, a gaming company based in Indianapolis–has needed to hire nine new workers since July, when he added the vaccine to his company’s job requirements. He wants to avoid compromising the safety of his existing 46 employees.

“We also emphasize that this is a good thing for the candidates who may soon work in the workplace,” says Holand, noting that employees who are vaccinated are at a lower risk of contracting Covid-19 than those who remain unvaccinated. “They are more protected and resistant to any potential infection of Covid and therefore their workdays won’t be affected,” he adds.

While Holand says that so far, all of the candidates he’s hired has agreed to GooseSmurfs’s vaccination policy and most of them already had their full dose. The founder suspects he’s had a smoother time with the process because the company has been transparent and direct with its requirements on the job post itself. He says most candidates who were hesitant about the mandate likely didn’t apply. “Overall, being straightforward about the policy made the hiring process easier and seamless,” he says.

Make the vaccine a condition of employment.

When people take a job, they do so with an understanding of a job’s requirements. As an employer, you don’t want to violate that contractual agreement because it could lead to turnover. That’s why it’s crucial to outline any vaccine policies with candidates before they accept the position, says John Hooker, professor of business ethics and social responsibility at Carnegie Mellon University.

Additionally, if you have a policy in which some workers are required to be vaccinated, such as those in the office, and others are not, that rationale should be clear upfront. “It’s critical to have these kind of [policies] run across the entire company, as opposed to allowing them for some people and not for others,” says Hooker.

And if you do require a vaccine for some and not others, Hooker suggests making your reasoning known: “There must be a reason for that distinction and it shouldn’t be arbitrary.” Employees are less likely to push back on policies when they understand the rationale behind them, he says.

Don’t ask about a prospective worker’s vaccination status.

If you have a mandate in place, you likely want to know whether you will have to accommodate a new employee who isn’t vaccinated. While it’s fine to ask about a person’s vaccination status, you can’t make your hiring decision based on that person’s status alone. If a candidate is turned down for a job, and is told it is because he or she won’t receive the vaccine, they can file a discrimination lawsuit.

It is illegal both under federal and state laws to discriminate against an employee based on his or her medical condition with regard to employment decisions. It is, however, difficult for applicants to prove that a company didn’t hire them because of a health condition, says Jared Pope, HR law specialist CEO of Work Shield, a Dallas-based HR software company.

If you do decide to pass on candidates after having a conversation about their vaccination status, be cordial. Thank them for applying and let them know that you’ll keep them in mind should a position open up that would be a better fit.

An even better idea? Don’t ask at all. Talk about the company’s policy regarding vaccines during the interview process. Let the candidate know if any exceptions can be made if they choose to move forward. “Questions about the workplace can be asked and answered in an interview, and are not discriminatory or illegal in nature,” says Pope. Down the line, you can require proof of vaccination, he adds.

Why Your Return to the Office Requires Two Workplace Safety Policies

Operating amid the pandemic has entered a new phase of difficulty–particularly for employers of both vaccinated and unvaccinated workers. Shortly after the CDC updated its guidelines on May 13, noting that vaccinated individuals no longer needed to wear facemasks indoors, the Occupational Safety and Health Administration (OSHA), a federal agency that oversees workplace health and safety, updated its Covid-19 guidance.

On June 26, OSHA updated guidance in compliance with the CDC to help employers protect workers who are still not vaccinated, with a special emphasis on industries with prolonged close-contacts such as meat processing, manufacturing, seafood, and grocery and high-volume retail. The guidance includes protocols for social distancing, mask wearing, and other health procedures meant to keep both parties safe.

Considering that just 52 percent of the U.S. population is fully vaccinated against the coronavirus, chances are some of your employees have yet to get a jab. That means if you’re planning a return to the office, you’ll also need to create two separate workplace health policies.

These policies will be different from business to business, depending on the level of community spread in a given location and the level of contact employees have with the public. But acting is a must, says David Barron, labor and employment attorney at Cozen O’Connor. Failing to address a stratified workplace–or even just relying on the honor system–could lead to legal trouble, a loss of morale, turnover, and employees falling sick.

Founders like Dominique Kemps aren’t taking any chances. Her business, GlassExpertsFL, a commercial glass repair company, is located in Miami. Florida overall has been particularly hard hit by the Delta variant, a more contagious strain of the coronavirus. Daily, about 10 in 100,000 people are contracting the coronavirus by way of the Delta variant. As of July 2, only 46 percent of the population of Florida was fully vaccinated, according to the CDC.

Kemps has devised two separate physical workspaces: one for vaccinated employees and another for those who remain unvaccinated. Also for unvaccinated employees, meetings are held virtually, while vaccinated employees can wear a mask and attend if desired. Vaccinated employees can also eat lunch together, while Kemps has asked unvaccinated employees to eat in a designated area. “Frankly,” she says, “it hasn’t been easy.”

Here’s how to ease the transition:

1. Request vaccination information.

Before you make any decisions regarding which policies to enact, first ask and keep track of who is vaccinated and who isn’t, says Dr. Shantanu Nundy, chief medical officer at Accolade, a benefit provider for health care workers. An employer can request a copy of an employee’s vaccination card or other proof, which should help you determine how much of your workforce falls under one policy or another.

If you opt to review vaccination information, note that anything you collect must be considered confidential information that has to be kept private in files that are separate from personnel files. A failure to do so may result in anti-discrimination violations under the Americans With Disabilities Act and the Genetic Information Nondiscrimination Act, two laws that protect workers from health status discrimination.

2. Overcommunicate any policy changes.

It’s also crucial to communicate any change in policy openly. Robert Johnson, founder of Sawinery, a Windsor, Connecticut-based creator of woodworking projects, divided workers into two shifts, the first for vaccinated individuals, and another for unvaccinated workers. He’s made it clear to his staff that he’s waiting until everyone is vaccinated before returning to the original schedule.

“The structure won’t compromise anyone’s safety and everyone can work without any worries in mind,” says Johnson.

3. Stay flexible.

If anything has been true about the pandemic, it’s that things can change rapidly. As such, Nundy recommends clarifying that policies are flexible and may be subject to change. Some unvaccinated folks may want to leave if they feel they’re being treated differently, such as not being allowed into the office. Some smart wording can easily allay these concerns, he says. Instead of telling unvaccinated employees that they’re not welcome in the office again, make it clear that the policies are temporary–if that’s the case, of course–and that you’re open to feedback, adds Nundy.

The occupational safety and health policy defines the goals for the occupational health and safety work in the workplace and for activities that promote the working capacity of the staff. The policy also describes occupational health and safety responsibilities and the way of organizing the cooperation measures. The preparation of the occupational safety and health policy is based on the Occupational Safety and Health Act. The policy is employer-specific and applies to all employers.

Workplace wellness is any workplace health promotion activity or organizational policy designed to support healthy behavior in the workplace and to improve health outcomes. Known as ‘corporate wellbeing’ outside the US, workplace wellness often comprises activities such as health education, medical screenings, weight management programs, on-site fitness programs or facilities.

Workplace wellness programs can be categorized as primary, secondary, or tertiary prevention efforts, or an employer can implement programs that have elements of multiple types of prevention. Primary prevention programs usually target a fairly healthy employee population, and encourage them to more frequently engage in health behaviors that will encourage ongoing good health (such as stress management, exercise and healthy eating).

Secondary prevention programs are targeted at reducing behavior that is considered a risk factor for poor health (such as smoking cessation programs and screenings for high blood pressure). Tertiary health programs address existing health problems (for example, by encouraging employees to better adhere to specific medication or self-managed care guidelines).
