Immunity Debt: Does It Really Exist?

The deaths of at least 190 people, including 30 children, from the invasive bacterial infection group A streptococcus, or strep A, are the most extreme consequences of a wave of winter infections that have seemingly left most of the country coughing and sneezing. The parlous state of the nation’s health has prompted suggestions that we are now paying an “immunity debt” incurred by the reduction of common infections during the Covid-19 lockdowns of 2020 and 2021.

But experts seem divided about whether the debt concept is genuine, let alone whether it explains the prevalence of non-Covid afflictions. As with so many of the debates about the outcomes of the pandemic, there do not appear to be simple answers – but no shortage of self-proclaimed “experts” ready to give them anyway.

While there are good reasons to believe that the measures taken to reduce the spread of the coronavirus have broader implications for common infectious diseases, there is no one-case-fits-all explanation for the spate of winter bugs, much less any obvious conclusions to be drawn about pandemic management.

Are the infection rates really so unusual anyway? Cases of strep A and associated scarlet fever certainly seem unusually high, with 37,068 recorded cases of the latter up to 8 January, compared to 4,490 at the same point in the earlier high season of 2017-18. But whether there is an anomalous prevalence of common infections across the board is less clear. “For some of the infections, we don’t really know,” says Petter Brodin, professor of paediatric immunology at Imperial College London.

Every winter, there is an upsurge in common infections such as flu, colds (caused by various viruses) and respiratory syncytial virus (RSV), which produces cold-like symptoms. Bacterial infections follow a similar seasonality, according to Michael Levin, professor of paediatrics and international child health at Imperial because they tend to take opportunistic advantage of an immune system weakened by a virus. RSV in infants and young children, for example, sometimes leads to bronchiolitis, an infection of the respiratory tract that can be serious.

At this time of year, “having hospital beds filled with children and adults with viral disease and pneumonia is not unusual”, says Levin. But although there is as yet little hard data, he adds that “there is an impression that we are having an unusually severe winter with RSV, flu and adenovirus”, the last of which causes cold-like symptoms. He adds: “We’re seeing increased numbers of children with severe bacterial infections – there is definitely more streptococcal infection around.”

There are similar reports across North America, while unusually high levels of RSV in children reported in Australia and New Zealand in mid-2021 provoked the first mentions of Covid immunity debt. Brodin agrees that something seems unusual both in the timing and the numbers of infections. In Sweden, where he was working until moving to Imperial in October 2021, the incidence of RSV was markedly unusual last year too.

If that is the case, what is the cause? “Here we enter the realm of guessing and speculation,” says Levin. It’s not clear that there is a general answer. “Every bug has a different dynamic,” concurs Brodin. Flu surges across the globe, driven largely by temperature changes and people’s travel behaviour, whereas RSV is present all the time in the population and usually peaks about November to December in the UK. The incidence of such infections also depends on how many children are born each year, since newborns have essentially no immunity.

Our immune systems need exposure to pathogens to maintain a good level of resistance. Many common infections such as colds are caused by endemic viruses and bacteria that people often carry asymptomatically because their immune systems keep the pathogen in check. For example, many people carry the meningococcal bacteria that cause meningitis, but only about one in 50,000 were, before vaccines, at risk of getting ill from them. “The normal state for most of these things is asymptomatic carriage,” says Levin.

Such symptom-free, low-level infection can prime the immune system to ward off another infection months or even years later. “Children and adults do build up a repertoire of immunity from exposure,” Levin says – as every parent knows, young children in particular are constantly exchanging common pathogens. But such immunity generally wanes, so if you haven’t been exposed for some time, for example, because of social distancing and lockdowns during 2020 and 2021, you will be more susceptible.

“It’s plausible that we may now have a population that has met fewer common viruses and is therefore more susceptible,” says Levin. Moreover, if fewer people have such immunity, there will be more transmission in the population. “Children normally catch scarlet fever in their first year at school, if at all,” says Shiranee Sriskandan, professor of infectious diseases at Imperial. “Scarlet fever rates plummeted during 2020-2021.

Therefore, school-age children may not have built up immunity to strep A, so we now have a much larger cohort of non-immune children.” A study last June reported that, because of reduced transmission of common respiratory viruses during the pandemic, breastfeeding infants were being passed fewer protective antibodies from the mother.

But whether there is a generalised immunity debt is less clear. How long post-infection immunity lasts varies from one virus to another. And for flu viruses, infection in one season may or may not offer protection in the next, depending on how closely related the two strains are. Levin says that in many cases we don’t really know how much repeated exposure is needed to sustain immunity – for some diseases, such as smallpox and measles, just one infection generally gives lifelong protection.

Another reason that has been proposed for enhanced infections this season is that Covid-19, which most of the population has now experienced at least once, has itself weakened some people’s immune systems. But this will probably not be widespread, if it happens at all. While there is evidence of immune dysfunction persisting for many months in some people who catch Covid, this is not so much a weakening of immunity as a problematic overactivation.

A study published last January by Prof Gail Matthews, an infectious diseases physician at the University of New South Wales in Australia, and her colleagues showed overactive immune function up to eight months after Covid infection in some people. “The most likely reason for this is some level of viral persistence that the immune system is recognising as foreign and still reacting to,” Matthews says. But she doubts that there is any evidence of widespread Covid-induced immune impairment.

Brodin has seen similar Covid-related immune dysfunction. He and his co-workers have studied children with multisystem inflammatory syndrome, a rare but serious condition in which an extreme response a few months after infection has to be treated with immunosuppressants. He says that for some people with long Covid too: “There is clearly something wrong with the way the immune system continues to be activated.”

But whether any such problems arise for the majority of people who have mild Covid symptoms is far less clear, he says. “There’s nothing that suggests that, although we can’t rule it out.” Lockdowns and social distancing do seem to have reduced infections such as flu and RSV during the pandemic. But if it turns out that those measures have contributed to higher rates of such infections now, what conclusions should we draw? Opponents of lockdowns say that it highlights another of their flaws, for which we are now paying a heavy price.

That position makes little sense. There is compelling evidence that lockdowns and social distancing saved lives, especially while vaccines were still not available. A day or two in bed with a bad cold seems a small price to pay for that. A small minority of researchers have, however, questioned whether lockdowns really made much difference to the spread of Covid.

Infectious-disease epidemiologist Sunetra Gupta of Oxford University asserted in the Daily Telegraph that lockdowns do little to slow the spread of an epidemic disease such as Covid-19 but have much more impact on endemic diseases such as colds and RSV. But Brodin is dismissive of such a crude division into epidemic versus endemic diseases. “It’s incredibly far-fetched to say that is the uniform solution,” he says.

Besides, he adds, how then to explain that RSV is also running riot now in Sweden, which famously eschewed lockdowns and kept schools open? Those who have noisily championed the Swedish approach to the pandemic can hardly argue that it would have made any difference to our alleged immunity debt now. In short, Brodin says, it is much too simplistic to say: “What we’re seeing in children’s hospitals is simply the result of lockdown.”

Besides, Levin adds: “Lockdown was an essential public health measure. It was the right strategy for a new disease with a totally susceptible population and probably saved thousands of lives.” A much more fruitful response to the surge in infections, says Brodin, is to “be thinking about how we develop better vaccines and get people to take them”. We should be developing and rolling out vaccines against flu (for which multistrain “universal” jabs are now being trialled) and RSV (for which a vaccine is on the near horizon) much faster, as well as thinking more seriously about non-pharmaceutical interventions such as wearing masks.

“It seems absolutely insane to me that we should get on a packed tube at rush hour and there will be people sneezing and coughing and not wearing a mask to protect others,” says Levin. “In Asian countries, you have always worn a mask if you have a cold.” He adds that we should also stop encouraging ill people to go to work and continue to advise hand washing and good hygiene. Flu, for instance, is spread mostly by people touching infected surfaces and then their eyes.

Such measures “could greatly reduce the burden of infection in the population”, he says. But vaccines are key. “Many of these severe diseases are vaccine-preventable and we should be ensuring that as many of the population as are susceptible are protected,” says Levin. If we rely on natural infection to build up immunity, though, isn’t that better than getting vaccinated? No, says Prof Peter Openshaw, who heads the infection programme of the Biomedical Research Centre at Imperial.

The immune response a vaccine awakens is “just as ‘natural’ as the response you get to the virus, sometimes even better, and certainly less dangerous”. “I can’t think of an example of a viral infection being better than getting vaccinated,” Openshaw adds. What’s more, the point of a vaccine is to protect those who would otherwise probably develop severe disease – and predicting who those people are is not always possible.

“We vaccinate all to protect the few, because we can’t identify the vulnerable,” says Levin. Covid should have taught us that. Casting this debate in terms of an immunity debt is therefore potentially misleading, as it implies that this is a zero-sum game: you have to keep paying with infections to stay in credit. Vaccines can simply cancel the “debt”. Who wouldn’t want that?

China Cuts Key Rates, Steps up Monetary Stimulus to Boost Economy

SHANGHAI (Reuters) -China lowered mortgage lending benchmark rates on Thursday as monetary authorities step up efforts to prop up the slowing economy, after data earlier in the week pointed to a darkening outlook for the country’s troubled property sector.

The cut to the one-year and five-year loan prime rates (LPR) followed surprise cuts by China’s central bank on Monday to its short- and medium-term lending rates, and came days after the central bank’s vice governor flagged more moves ahead.

With the property sector’s downturn seen persisting into 2022 and the fast-spreading Omicron variant dampening consumer activity, many analysts say those easing measures will be necessary, even as other major economies, including the United States, appear set to tighten monetary policy this year.

December economic data showed further weakening in consumption and the property sector, both major growth drivers.

At a monthly fixing on Thursday, China lowered its one-year loan prime rate (LPR) by 10 basis points to 3.70% from 3.80%. The five-year LPR was reduced by 5 basis points to 4.60% from 4.65%, its first cut since April 2020.

China’s central bank “should hurry up, make our operations forward-looking, move ahead of the market curve, and respond to the general concerns of the market in a timely manner,” People’s Bank of China Vice Governor Liu Guoqiang said on Tuesday, heightening market expectations for more stimulus.

All 43 participants in a snap Reuters poll had predicted a cut to the one-year LPR for a second straight month. Among them, 40 respondents also forecast a reduction in the five-year rate.

The cut to the 5-year rate suggested that “the Chinese authorities are keen to lower the cost of credit lending, so total credit growth is expected to rebound after the Spring Festival to ease the pressure on macro economy,” said Marco Sun, chief financial analyst at MUFG.

“China’s monetary policy still has some room for easing in the first half of this year, depending on the policy transmission effect and the growth target set by annual parliamentary meeting in March.”

Property firms’ shares and bonds jumped on Thursday following the LPR cut, as investors hoped it and other recent government measures would help to ease a funding squeeze in the sector that has seen a growing number of developers default on their debts.

Sheana Yue, China economist at Capital Economics, expects a further 20 bps cut to the one-year LPR in the first half of this year.

Interest rates on medium-term lending facilities (MLF) serve as a guide to the LPR. Market participants believe moves to the LPR should mimic adjustments to MLF rates.

The outlook for the global economy darkened as a stream of data from Europe and Asia suggested growth faltered in the third quarter, hobbled by world-wide supply-chain snarls, sharply accelerating inflation and the impact of the highly contagious Delta variant.

U.S. inflation accelerated last month and remained at its highest rate in over a decade, with price increases from pandemic-related labor and materials shortages rippling through the economy from a year earlier.

The Labor Department said last month’s consumer-price index, which measures what consumers pay for goods and services, rose by 5.4%

The gap between yields on shorter- and longer-term Treasury’s narrowed Wednesday after data showed inflation accelerated slightly in September, fueled by investors’ bets that the Federal Reserve may need to tighten monetary policy sooner than expected. Measures of inflation in China and the U.S. highlight this week’s economic data.

China’s exports, long a growth engine for the country’s economy, are expected to increase 21% from a year earlier in September, according to economists polled by The Wall Street Journal. That is down from a 25.6% gain in August. Meanwhile, inbound shipments are forecast to rise 19.1% from a year earlier, retreating from the 33.1% jump in August.

The International Monetary Fund releases its World Economic Outlook report during annual meetings. The latest forecasts are likely to underscore the relatively quick economic rebound of advanced economies alongside a slower recovery in developing nations with less access to Covid-19 vaccines.

China’s factory-gate prices for September are expected to surge 10.4% from a year earlier, a pace that would surpass its previous peak in 2008, according to economists polled by The Wall Street Journal. Higher commodity costs have led to the rise in producer prices this year, but so far that hasn’t fed through to consumer inflation. Economists forecast the consumer-price index rose only 0.7% from a year earlier in September.

September’s U.S. consumer-price index is expected to show inflation remained elevated as companies passed along higher costs for materials and labor. Rising energy prices likely contributed to the headline CPI, while core prices, which exclude food and energy, might start to reflect climbing shelter costs.

The Federal Reserve releases minutes from its September meeting, potentially offering additional insight on plans to start reducing pandemic-related stimulus.

U.S. jobless claims are forecast to fall for the second consecutive week as employers hold on to workers in a tight labor market. The data on claims, a proxy for layoffs, will cover the week ended Oct. 9.

U.S. retail sales are expected to fall in September. U.S. consumers appear to be in decent financial shape, but Covid-related caution, rising prices and widespread supply-chain disruptions are tamping down purchases. The auto industry has been especially hard hit by a semiconductor shortage—separate data released earlier this month show U.S. vehicle sales in September fell to their lowest level since early in the pandemic.

Scan QR Code Menus With a Side of Caution, Say Privacy Experts

Restaurant patrons who’ve grown accustomed during the pandemic to whipping out their phones to access menus using QR codes should understand the implications for their personal data, say privacy and cyber-security experts.

That’s especially important given some restaurant owners are finding electronic menus efficient and cost effective, and that they may hold onto the practice even after COVID-19 is more contained.

It’s not the QR code itself that collects customer data, said Dustin Moores, a privacy lawyer with nNovation LLP in Ottawa.

“What the QR code does is it sort of acts as a web link to a web page. So when you scan a QR code on your phone, in all likelihood it is going to send you to either the restaurant’s website, or to the website of a service provider that’s being used by the restaurant,” he told Cost of Living producer Jennifer Keene.

“What’s happening is we’re replacing a very sort of innocuous object, a restaurant menu, with a website that comes with all the sort of tracking technologies that you see in modern e-commerce today.”

A marketing device

Bringing up an online menu on your phone doesn’t mean you’re handing data such as your birth date and banking details to bad actors on the internet.

The more immediate implication is that it gives your local pub, or the platform they use, new knowledge of your behaviours and preferences that it can use to better sell to you.

“If you’re a returning customer to to one of these restaurants that use the QR code technology, they might be able to say, ‘Hey, we know that Jennifer ordered the Caesar salad last time; let’s put it at the top of our menu this time because we know that she likes it,'” said Moores.

The restaurant could also use the information it has gathered to upsell customers, such as suggesting the customer add chicken to that salad, he said. Ot it could try to influence your choices by offering a discount on the dish you enjoyed last time.

Moore said it’s also likely that the QR code will take you to a website that uses third-party cookies that can be used to track your web browsing habits. “Let’s say it was a Hungarian restaurant that you visited. Well then other Hungarian restaurants in the area might start advertising to you all of a sudden,” he said.

An issue of consent

Moore said his biggest legal concern about the spike in use of QR code-enabled menus is consent.

“I think what might get lost on a lot of restaurant owners is that, like every other business in Canada, they’re subject to our privacy laws,” he said. “Whenever a business collects, uses or shares personal information in the course of commercial activities, they need to have people’s consent to do that.”

Cyber-security expert Yuan Stevens, policy lead for technology, cyber-security and democracy at Ryerson University’s Leadership Lab, said the security concerns related to QR codes remain “fairly hypothetical.”

“I have not yet found any cases in Canada of QR codes being used for stealing data or violating your privacy,” she said. “But I also think it is useful to keep in mind what concerns we should be aware of as technology becomes ubiquitous.”

Someone who wants to direct you to a malicious website could “fast track” that process using a QR code, said Stevens. “Phishing and scams are already happening. And QR codes would just be another conduit to that.”

She said some restaurants are using QR codes to gather contact tracing information as well as for menus.

With the drive to reduce contact with surfaces and each other, QR codes have increased in popularity during the pandemic, said Stevens, particularly in China, where their use increased six per cent between 2019 and 2020.

Stevens notes that last month a benevolent hacking group already alerted the public that it had been able to hack the Quebec government’s new vaccine passport system, which led to 300,000 QR codes being exposed. The developer resolved the issue within 24 hours, but it’s good to be aware that there are privacy and security tradeoffs that come with using technology, she said.

QR-code enabled vaccination verification systems are now in place in Manitoba and New Brunswick, and will be in Ontario as of Oct. 22.

Jenny Burthwright, owner of Jane Bond BBQ in Calgary, said her business introduced QR code menus in the fall of 2020 when they’d been “ripping through” paper menus while trying to keep COVID-safe.She plans to keep the higher-tech system in place post-pandemic.

“There’s a very obvious cost savings to it,” she said. “With the rising costs of everything, we considered that, and also environmentally just wanted to move away from that paper.

Restaurants are also finding it easier and faster to update an online menu than a printed one, said Olivier Bourbeau, a vice-president of Restaurants Canada, the industry association representing food-service employers.

Being able to quickly add or remove a menu item, or update the price of the dish, is particularly useful given the complexities of running a food-service business during this crisis, including rising food costs and supply-chain problems that delay delivery of ingredients.

Those advantages will likely mean many restaurants will keep the QR-code system in place, Bourbeau said.

Protective measures

To mediate the risks associated with leaving a digital trail every time you order a brisket sandwich or a poke bowl, there are some precautions consumers can take, according to cyber-security expert Stevens.

The same principles that you’d apply to avoiding phishing and other online scams generally also apply to using QR codes, she said.

“Be careful of offers that seem too good to be true. Don’t give sensitive information over email or phone to untrusted sources. Be careful what you click on.”

Treat a QR code with the same care as an email attachment, and keep your eyes peeled for printed QR codes that look like they’ve been duplicated — one stuck on top of another, said Stevens.

It’s worth taking the time to check with your host or server to make sure the QR code you’re about to use is legit, she said.

“You want to be really careful that the QR code you’re scanning is actually the restaurant’s, otherwise you could be misled. And that’s when you’d be scammed.
