Allergic To The World: Can Medicine Help People With Severe Intolerance To Chemicals

Sharon calls herself a universal reactor. In the 1990s, she became allergic to the world, to the mould colonising her home and the paint coating her kitchen walls, but also deodorants, soaps and anything containing plastic. Public spaces rife with artificial fragrances were unbearable. Scented disinfectants and air fresheners in hospitals made visiting doctors torture. The pervasiveness of perfumes and colognes barred her from in-person social gatherings.

Even stepping into her own back garden was complicated by the whiff of pesticides and her neighbour’s laundry detergent sailing through the air. When modern medicine failed to identify the cause of Sharon’s illness, exiting society felt like her only solution. She started asking her husband to strip and shower every time he came home. Grandchildren greeted her through a window. When we met for the first time, Sharon had been housebound for more than six years.

When I started medical school, the formaldehyde-based solutions used to embalm the cadavers in the human anatomy labs would cause my nose to burn and my eyes to well up – representing the mild, mundane end of a chemical sensitivity spectrum. The other extreme of the spectrum is an environmental intolerance of unknown cause (referred to as idiopathic by doctors) or, as it is commonly known, multiple chemical sensitivity (MCS).

An official definition of MCS does not exist because the condition is not recognised as a distinct medical entity by the World Health Organization or the American Medical Association, although it has been recognised as a disability in countries such as Germany and Canada.

Disagreement over the validity of the disease is partially due to the lack of a distinct set of signs and symptoms, or an accepted cause. When Sharon reacts, she experiences symptoms from seemingly every organ system, from brain fog to chest pain, diarrhoea, muscle aches, depression and odd rashes. There are many different triggers for MCS, sometimes extending beyond chemicals to food and even electromagnetic fields. Consistent physical findings and reproducible lab results have not been found and, as a result, people such as Sharon not only endure severe, chronic illness but also scrutiny over whether their condition is “real”.

The first reported case of MCS was published in the Journal of Laboratory and Clinical Medicine in 1952 by the American allergist Theron Randolph. Although he claimed to have previously encountered 40 cases, Randolph chose to focus on the story of one woman, 41-year-old Nora Barnes. She had arrived at Randolph’s office at Northwestern University in Illinois with a diverse and bizarre array of symptoms.

A former cosmetics salesperson, she represented an “extreme case”. She was always tired, her arms and legs were swollen, and headaches and intermittent blackouts ruined her ability to work. A doctor had previously diagnosed her with hypochondria, but Barnes was desperate for a “real” diagnosis.

Randolph noted that the drive into Chicago from Michigan had worsened her symptoms, which spontaneously resolved when she checked into her room on the 23rd floor of a hotel where, Randolph reasoned, she was far away from the noxious motor exhaust filling the streets. In fact, in his report Randolph listed 30 substances that Barnes reacted to when touched (nylon, nail polish), ingested (aspirin, food dye), inhaled (perfume, the “burning of pine in fireplace”) and injected (the synthetic opiate meperidine, and Benadryl).

He posited that Barnes and his 40 other patients were sensitive to petroleum products in ways that defied the classic clinical picture of allergies. That is, rather than an adverse immune response, such as hives or a rash where the body is reacting to a particular antigen, patients with chemical sensitivities were displaying an intolerance. Randolph theorised that, just as people who are lactose-intolerant experience abdominal pain, diarrhoea and gas because of undigested lactose creating excess fluid in their gastrointestinal tract, his patients were vulnerable to toxicity at relatively low concentrations of certain chemicals that they were unable to metabolise.

He even suggested that chemical sensitivity research was being suppressed by “the ubiquitous distribution of petroleum and wood products”. MCS, he believed, was not only a matter of scientific exploration, but also of deep-seated corporate interest. Randolph concludes his report with his recommended treatment: avoidance of exposure.

In that one-page abstract, Randolph cut the ribbon on the completely novel but quickly controversial field of environmental medicine. Nowadays, we hardly question the ties between the environment and wellbeing. The danger of secondhand smoke, the realities of climate change and the endemic nature of respiratory maladies such as asthma are common knowledge. The issue was that Randolph’s patients lacked abnormal test results (specifically, diagnostic levels of immunoglobulin E, a blood marker that is elevated during an immune response). Whatever afflicted them were not conventional allergies, so conventional allergists resisted Randolph’s hypotheses.

Randolph was in the dark. Why was MCS only now rearing its head? He also asked another, more radical question: why did this seem to be a distinctly American phenomenon? After all, the only other mention of chemical sensitivities in medical literature was in the US neurologist George Miller Beard’s 1880 textbook A Practical Treatise on Nervous Exhaustion (Neurasthenia).

Beard argued that sensitivity to foods containing alcohol or caffeine was associated with neurasthenia, a now-defunct term used to describe the exhaustion of the nervous system propagated by the US’s frenetic culture of productivity. Like Beard, Randolph saw chemical sensitivities as a disease of modernity, and conceived the origin as wear-and-tear as opposed to overload.

Randolph proposed that Americans, propelled by the post-second world war boom, had encountered synthetic chemicals more and more in their workplaces and homes, at concentrations considered acceptable for most people. Chronic exposure to these subtoxic dosages, in conjunction with genetic predispositions, strained the body and made patients vulnerable. On the back of this theory, Randolph developed a new branch of medicine and, with colleagues, founded the Society for Clinical Ecology, now known as the American Academy of Environmental Medicine.

As his professional reputation teetered, his popularity soared and patients flocked to his care. Despite this growth in interest, researchers never identified blood markers in MCS patients, and trials found that people with MCS couldn’t differentiate between triggers and placebos. By 2001, a review in the Journal of Internal Medicine found MCS virtually nonexistent outside western industrialised countries, despite the globalisation of chemical use, suggesting that the phenomenon was culturally bound.

MCS subsequently became a diagnosis of exclusion, a leftover label used after every other possibility was eliminated. The empirical uncertainty came to a head in 2021, when Quebec’s public health agency, the INSPQ, published an 840-page report that reviewed more than 4,000 articles in the scientific literature, concluding that MCS is an anxiety disorder.

In medicine, psychiatric disorders are not intrinsically inferior; serious mental illness is, after all, the product of neurological dysfunction. But the MCS patients I spoke to found the language offensive and irresponsible. Reducing what they felt in their eyes, throats, lungs and guts to anxiety was not acceptable at all.

As a woman I will call Judy told me: “I would tell doctors my symptoms, and then they’d run a complete blood count and tell me I looked fine, that it must be stress, so they’d shove a prescription for an antidepressant in my face and tell me to come back in a year.” In fact, because MCS is so stigmatising, such patients may never receive the level of specialised care they need.

In the wake of her “treatment”, Judy was frequently bedbound from crushing fatigue, and no one took her MCS seriously. “I think a lot of doctors fail to understand that we are intelligent,” she said. “A lot of us with chemical sensitivities spend a good amount of our time researching and reading scientific articles and papers. I probably spent more of my free time reading papers than most doctors.”

Judy grew up in Texas, where she developed irritable bowel syndrome and was told by doctors that she was stressed. Her 20s were spent in Washington state where she worked as a consultant before a major health crash left her bedbound for years (again, the doctors said she was stressed). Later, after moving to Massachusetts, a new paint job at her home gave her fatigue and diarrhoea.

She used to browse the local art museum every Saturday, but even fumes from the paintings irritated her symptoms. She visited every primary care doctor in her city, as well as gastroenterologists, cardiologists, neurologists, endocrinologists and even geneticists. Most of them reacted the same way: with a furrowed brow and an antidepressant prescription in hand. “Not one allopathic doctor has ever been able to help me,” Judy said.

Morton Teich is one of the few physicians who diagnoses and treats patients with MCS in New York. The entrance to his integrative medicine private practice is hidden away behind a side door in a grey-brick building on Park Avenue. As I entered the waiting room, the first thing to catch my eye was the monstrous mountain of folders and binders precariously hugging a wall, in lieu of an electronic medical record.

I half-expected Teich’s clinic to resemble the environmental isolation unit used by Randolph in the 1950s, with an airlocked entrance, blocked ventilation shafts and stainless-steel air-filtration devices, books and newspapers in sealed boxes, aluminium walls to prevent electromagnetic pollution, and water in glass bottles instead of a cooler. But there were none of the above. The clinic was like any other family medicine practice I had seen before; it was just very old. The physical examination rooms had brown linoleum floors and green metal chairs and tables. And there were no windows.

Although several of Teich’s patients were chemically sensitive, MCS was rarely the central focus of visits. When he introduced me, as a student writing about MCS, to his first patient of the day, a petrol-intolerant woman whose appointment was over the phone because she was housebound, she admitted to never having heard of the condition. “You have to remember,” Teich told me, “that MCS is a symptom. It’s just one aspect of my patients’ problems. My goal is to get a good history and find the underlying cause.” Later, when I asked him whether he had observed any patterns suggesting an organic cause of MCS, he responded: “Mould. Almost always.”

Many people with MCS I encountered online also cited mould as a probable cause. Sharon told me about her first episode in 1998, when she experienced chest pain after discovering black mould festering in her family’s trailer home. A cardiac examination had produced no remarkable results, and Sharon’s primary care physician declared that she was having a panic attack related to the stress of a recent miscarriage. Sharon recognised that this contributed to her sudden health decline, but also found that her symptoms resolved only once she began sleeping away from home.

She found recognition in medical books such as Toxic (2016) by Neil Nathan, a retired family physician who argued that bodily sensitivities were the product of a hyper-reactive nervous system and a vigilant immune system that fired up in reaction to toxicities, much as Randolph had said. The conditions that Nathan describes are not supported by academic medicine as causes of MCS: mould toxicity and chronic Lyme disease are subject to the same critique.

Sharon went to see William Rea, a former surgeon (and Teich’s best friend). Rea diagnosed her with MCS secondary to mould toxicity. “Mould is everywhere,” Teich told me. “Not just indoors. Mould grows on leaves. That’s why people without seasonal allergies can become chemically sensitive during autumn.” When trees shed their leaves, he told me, mould spores fly into the air. He suspected that American mould is not American at all, but an invasive species that rode wind currents over the Pacific from China. He mentioned in passing that his wife recently died from ovarian cancer. Her disease, he speculated, also had its roots in mould.

In fact, Teich commonly treats patients with nystatin, an antifungal medication used to treat candida yeast infections, which often infect the mouth, skin and vagina. “I have an 80% success rate,” he told me. I was dubious that such a cheap and commonplace drug was able to cure an illness as debilitating as MCS, but I could not sneer at his track record. Every patient I met while shadowing Teich was comfortably in recovery, with smiles and jokes, miles apart from the people I met in online support groups who seemed to be permanently in the throes of their illness.

However, Teich was not practising medicine as I was taught it. This was a man who believed that the recombinant MMR vaccine could trigger “acute autism” – traditionally an anti-science point of view. When one of his patients, a charismatic bookworm I’ll call Mark, arrived at an appointment with severe, purple swelling up to his knees and a clear case of stasis dermatitis (irritation of the skin caused by varicose veins), Teich reflexively blamed mould and wrote a prescription for nystatin instead of urging Mark to see a cardiologist.

When I asked how a fungal infection in Mark’s toes could cause such a bad rash on his legs, he responded: “We have candida everywhere, and its toxins are released into the blood and travel to every part of the body. The thing is, most people don’t notice until it’s too late.” Moulds and fungi are easy scapegoats for inexplicable illnesses because they are so ubiquitous in our indoor and outdoor environments. A great deal of concern over mould toxicity (or, to use the technical term, mycotoxicosis) stems from the concept of “sick-building syndrome”, in which visible black mould is thought to increase sensitivity and make people ill.

This was true of Mark, who could point to the demolition of an old building across the street from his apartment as a source of mould in the atmosphere. Yet in mainstream medicine, diseases caused by moulds are restricted to allergies, hypersensitivity pneumonitis (an immunologic reaction to an inhaled agent, usually organic, within the lungs) and infection.

Disseminated fungal infections occur almost exclusively in patients who are immunocompromised, hospitalised or have an invasive foreign body such as a catheter. Furthermore, if “clinical ecologists” such as Teich are correct that moulds such as candida can damage multiple organs, then it must be spreading through the bloodstream. But I have yet to encounter a patient with MCS who reported fever or other symptoms of sepsis (the traumatic, whole-body reaction to infection) as part of their experience.

Teich himself did not use blood cultures to verify his claims of “systemic candidiasis”, and instead looked to chronic fungal infection of the nails, common in the general population, as sufficient proof.

“I don’t need tests or blood work,” he told me. “I rarely ever order them. I can see with my eyes that he has mould, and that’s enough.” It was Teich’s common practice to ask his patients to remove their socks to reveal the inevitable ridges and splits on their big toenails, and that’s all he needed.

Through Teich, I met a couple who were both chemically sensitive but otherwise just regular people. The wife, an upper-middle-class white woman I will call Cindy, had a long history of allergies and irritable bowel syndrome. She became ill whenever she smelled fumes or fragrances, especially laundry detergent and citrus or floral scents. Teich put both her and her husband on nystatin, and their sensitivities lessened dramatically.

What struck me as different about her case, compared with other patients with MCS, was that Cindy was also on a course of antidepressants and cognitive behavioural therapy, the standard treatment for anxiety and depression. “It really helps to cope with all the stress that my illness causes. You learn to live despite everything,” she said.

In contemporary academic medicine, stress and anxiety cause MCS, but MCS can itself cause psychiatric symptoms. Teich later told me, unexpectedly, that he had no illusions about whether MCS is a partly psychiatric illness: “Stress affects the adrenals, and that makes MCS worse. The mind and the body are not separate. We have to treat the whole person.”

To understand this case, I also spoke to Donald Black, associate chief of staff for mental health at the Iowa City Veterans Administration Health Care. He co-authored a recent article on idiopathic environmental intolerance that took a uniform stance on MCS as a psychosomatic disorder. In 1988, when Black was a new faculty member at the University of Iowa, he interviewed a patient entering a drug trial for obsessive-compulsive disorder.

He asked the woman to list her medications, and watched as she started unloading strange supplements and a book about environmental illness from her bag. The woman had been seeing a psychiatrist in Iowa City – a colleague of Black’s – who had diagnosed her with systemic candidiasis. Black was flummoxed. If that diagnosis was true, then the woman would be very ill, not sitting calmly before him.

Besides, it was not up to a psychiatrist to treat a fungal infection. How did he make the diagnosis? Did he do a physical or run blood tests? No, the patient told him, the psychiatrist just said that her symptoms were compatible with candidiasis. These symptoms included chemical sensitivities. After advising the patient to discard her supplements and find a new psychiatrist, Black made some phone calls and discovered that, indeed, his colleague had fallen in with the clinical ecologists.

Black was intrigued by this amorphous condition that had garnered an endless number of names: environmentally induced illness, toxicant-induced loss of tolerance, chemical hypersensitivity disease, immune dysregulation syndrome, cerebral allergy, 20th-century disease, and mould toxicity. In 1990, he solicited the aid of a medical student to find 26 subjects who had been diagnosed by clinical ecologists with chemical sensitivities and to conduct an “emotional profile”.

Every participant in their study filled out a battery of questions that determined whether they satisfied any of the criteria for psychiatric disorders. Compared with the controls, the chemically sensitive subjects had 6.3 times higher lifetime prevalence of major depression, and 6.8 times higher lifetime prevalence of panic disorder or agoraphobia; 17% of the cases met the criteria for somatisation disorder (an extreme focus on physical symptoms – such as pain or fatigue – that causes major emotional distress and problems functioning).

In my own review of the literature, it was clear that the most compelling evidence for MCS came from case studies of large-scale “initiating events” such as the Gulf war (where soldiers were uniquely exposed to pesticides and pyridostigmine bromide pills to protect against nerve agents) or the terrorist attacks on the US of 11 September 2001 (when toxins from the falling towers caused cancers and respiratory ailments for years).

In both instances, a significant number of victims developed chemical intolerances compared with populations who were not exposed. From a national survey of veterans deployed in the Gulf war, researchers found that up to a third of respondents reported multi-symptom illnesses, including sensitivity to pesticides – twice the rate of veterans who had not deployed.

Given that Gulf war veterans experienced post-traumatic stress disorder at levels similar to those in other military conflicts, the findings have been used to breathe new life into Randolph’s idea of postindustrial toxicities leading to intolerance. The same has been said of the first responders and the World Trade Centre’s nearby residents, who developed pulmonary symptoms when exposed to “cigarette smoke, vehicle exhaust, cleaning solutions, perfume, or other airborne irritants” after 9/11, according to a team at Mount Sinai.

Black, who doubts a real disease, has no current clinical experience with MCS patients. (Apart from the papers he wrote more than 20 years ago, he had seen only a handful of MCS patients over the course of his career.) Despite this, he had not only written the article about MCS, but also a guide in a major online medical manual on how to approach MCS treatment as a psychiatric disease. When I asked him if there was a way for physicians to regain the trust of patients who have been bruised by the medical system, he simply replied:

“No.” For him, there would always be a subset of patients who are searching for answers or treatments that traditional medicine could not satisfy. Those were the people who saw clinical ecologists, or who left society altogether. In a time of limited resources, these were not the patients on which Black thought psychiatry needed to focus. It became clear to me why even the de facto leading professional on MCS had hardly any experience actually treating MCS.

In his 1990 paper, Black – then a young doctor – rightly observed that “traditional medical practitioners are probably insensitive to patients with vague complaints, and need to develop new approaches to keep them within the medical fold.  The study subjects clearly believed that their clinical ecologists had something to offer them that others did not: sympathy, recognition of pain and suffering, a physical explanation for their suffering, and active participation in medical care.”I wondered if Black had given up on these “new approaches” because few CS patients wanted to see a psychiatrist in the first place.

Physicians on either side of the debate agreed that mental illness is a crucial part of treating MCS, with one I spoke to believing that stress causes MCS, and another believing that MCS causes stress. To reconcile the views, I interviewed another physician, Christine Oliver, a doctor of occupational medicine in Toronto, where she has served on the Ontario Task Force on Environmental Health. Oliver believes that both stances are probably valid and true. “No matter what side you’re on,” she told me, “there’s a growing consensus that this is a public health problem.”

Oliver represents a useful third position, one that takes the MCS illness experience seriously while sticking closely to medical science. As one of few “MCS-agnostic” physicians, she believes in a physiological cause for MCS that we cannot know and therefore cannot treat directly due to lack of research. Oliver agrees with Randolph’s original suggestion of avoiding exposures, although she understands that this approach has resulted in traumatising changes in patients’ abilities to function. For her, the priority for MCS patients is a practical one: finding appropriate housing.

Often unable to work and with a limited income, many of her patients occupy public housing or multi-family dwellings. The physician of an MCS patient must act like a social worker. Facilities such as hospitals, she feels, should be made more accessible by reducing scented cleaning products and soaps. Ultimately, finding a non-threatening space with digital access to healthcare providers and social support is the best way to allow the illness to run its course.

Whether organic or psychosomatic or something in between, MCS is a chronic illness. “One of the hardest things about being chronically ill,” wrote the American author Meghan O’Rourke in the New Yorker in 2013 about her battle against Lyme disease, “is that most people find what you’re going through incomprehensible – if they believe you are going through it. In your loneliness, your preoccupation with an enduring new reality, you want to be understood in a way that you can’t be.”

A language for chronic illness does not exist beyond symptomatology, because in the end symptoms are what debilitate “normal” human functioning. In chronic pain, analgesics can at least deaden a patient’s suffering. The same cannot be said for MCS symptoms, which are disorienting in their chaotic variety, inescapability and inexpressibility. There are few established avenues for patients to completely avoid triggering their MCS, and so they learn to orient their lives around mitigating symptoms instead, whether that is a change in diet or moving house, as Sharon did. MCS comes to define their existence.

As a housebound person, Sharon’s ability to build a different life was limited. Outside, the world was moving forward, yet Sharon never felt left behind. What allowed her to live with chronic illness was not medicine or therapy, but the internet. On a typical day, Sharon wakes up and prays in bed. She wolfs down handfuls of pills and listens to upbeat music on YouTube while preparing her meals for the day: blended meats and vegetables, for easier swallowing.

The rest of the day is spent on her laptop computer, checking email and Facebook, watching YouTube videos until her husband returns home in the evening. Then bed. This is how Sharon has lived for the past six years, and she does not expect anything different from the future. When I asked her if being homebound was lonely, I was taken aback at her reply: “No.”

In spite of not having met most of her 15 grandchildren (with two more on the way), Sharon keeps in daily contact with all of them. In fact, Sharon communicates with others on a nearly constant basis. “Some people are very much extroverts,” Sharon wrote. “I certainly am. But there are also people who need physical touch … and I can understand why they might need to see ‘real people’ then … but it’s very possible to be content with online friends. This is my life!”

The friendships that Sharon formed online with other housebound people with chronic illnesses were the longest-lasting and the most alive relationships she had ever known. She had never met her best friend of 20 years – their relationship existed completely through letters and emails, until two years ago, when the friend died. That “was very hard for me”, Sharon wrote.

The pandemic changed very little of Sharon’s life. If anything, Covid-19 improved her situation. Sharon’s local church live-streamed Sunday service, telehealth doctor appointments became the default, YouTube exploded in content, and staying indoors was normalised. Sharon saw her network steadily expand as more older adults became isolated in quarantine.

People within the online MCS community call themselves “canaries”, after the birds historically used as sentinels in coalmines to detect toxic levels of carbon monoxide. With a higher metabolism and respiratory rate, the small birds would theoretically perish before the less-sensitive human miners, providing a signal to escape. The question for people with MCS is: will anyone listen?

“Us canaries,” said a woman named Vera, who was bedbound from MCS for 15 years after a botched orthopaedic surgery, “we struggle and suffer in silence.” Now, in the information age, they have colonised the internet to find people like themselves. For our part, we must reimagine chronic illness – which will become drastically more common in the aftermath of the pandemic – where what matters to the patient is not only a scientific explanation and a cure, but also a way to continue living a meaningful life.

This calls into action the distinction between illness and disease that the psychiatrist and anthropologist Arthur Kleinman made in his 1988 book The Illness Narratives. Whereas a disease is an organic process within the body, illness is the lived experience of bodily processes. “Illness problems,” he writes, “are the principal difficulties that symptoms and disability create in our lives.”

By centring conversations about MCS on whether or not it is real, we alienate the people whose illnesses have deteriorated their ability to function at home and in the world. After all, the fundamental mistrust does not lie in the patient-physician relationship, but between patients and their bodies. Chronic illness is a corporeal betrayal, an all-out assault on the coherent self. Academic medicine cannot yet shed light on the physiological mechanisms that would explain MCS. But practitioners and the rest of society must still meet patients with empathy and acceptance, making space for their narratives, their lives, and their experience in the medical and wider world.

After two years of wrangling, false starts and disappointments, it finally happened: America has passed its first-ever climate legislation, moving the country closer to its goal of a decarbonized future and taking a significant step toward helping the planet avert the worst scenarios of climate catastrophe.

But it’s not a time to rest. We have always held power to account – on climate and  every other major issue – from the fossil fuel companies responsible for heating the planet to the politicians representing their interests. The country responsible for the most greenhouse gas emissions in history has indicated it will change course; we will relentlessly report on what comes next, who will benefit and the remaining obstacles to progress.

Increased Physical Activity Reduces Breast Cancer Risk, International Study Suggests

Increasing physical activity and reducing time spent sedentary is likely to decrease the risk of breast cancer, a study of more than 100,000 women suggests. An international team including researchers from Australia, the UK and US have used genetic analysis to establish a causal relationship between overall activity levels and cancer risk.

The study, published in the British Journal of Sports Medicine, included data from 130,957 women, 76,505 of whom had breast cancer. While previous research has shown a correlation between physical activity and lowered risk of breast cancer, proving causation has been difficult.

“There has always been a little bit of uncertainty as to whether physical activity truly causes a lower rate of breast cancer or whether that relationship is confounded by other factors,” said Associate Prof Brigid Lynch of Cancer Council Victoria, the study’s senior author.

“For example, women who tend to be more physically active might have healthier lifestyles in other ways as well.”Lynch said the new study “suggests that it is certainly a causal effect: physical activity does reduce your risk of developing breast cancer”.“We saw a risk reduction across all breast cancer types,” she said.

The researchers used a technique known as Mendelian randomisation to establish causality, which uses genetic variants as proxies for particular traits – in this case, physical activity and time spent sitting or reclining.They found that a higher level of physical activity, or general movement, was associated with a 41% reduction in invasive breast cancer risk.

In pre- and perimenopausal women, vigorous physical activity at least three days per week was linked to a 38% lower risk of breast cancer compared to no vigorous activity.“For every 100 odd minutes [of sedentary time] per day we observed a 20% risk increase in breast cancer overall, and a doubling of risk for triple-negative breast cancer [which is more aggressive and difficult to treat],” Lynch said.

Physical activity is thought to lower breast cancer risk because it decreases the amount of both oestrogen and androgen hormones circulating in the bloodstream. A reduction in inflammation may also be a factor.“There’s always been a lot of focus on other health behaviours like healthy eating, maintaining a healthy weight, reducing alcohol intake,” Lynch said. “There’s a big role for physical activity in preventing cancer.”

The research drew data from the UK Biobank as well as 76 other studies conducted as part of the Breast Cancer Association Consortium.One limitation of the study was that it only included data from women of European ancestry.“We can’t say for sure that these genetic instruments are applicable across different racial backgrounds,” Lynch said.

But she added that large observational studies in Asia and for women of different ethnicities in the US have found correlations between physical activity and reduced risk.“We do already recommend that physical activity is one of things you can do to reduce your breast cancer risk,” said Associate Prof Wendy Ingman of the University of Adelaide, who was not involved in the study.

Other factors associated with a reduced risk of breast cancer include minimising alcohol intake, and breastfeeding. “The longer a woman breastfeeds for, the less breast cancer risk she has,” Ingman said.

A population-based case-control study was conducted with 199 women (97 cases and 102 controls) from the Washington, DC metro area. A self-report physical activity questionnaire elicited responses on walking for exercise and vigorous physical activity (e.g., running, aerobics, etc.) in the past year. Responses were used to calculate a metabolic equivalent (MET) score [MET-hours/week = hours/week vigorous activity×7+ hours/week walking ×3]. The MET score was categorized into low, medium and high tertiles. Multivariate logistic regression examined the association between physical activity and breast cancer.

African American women who engaged in vigorous physical activity (> 2 hours/week in the past year) had a 64% reduced risk of breast cancer compared to those who did not participate in any vigorous activity (odds ratio, OR = 0.36; 95% confidence interval, CI = 0.17–0.75). We also found a 64% reduced breast cancer risk in women with a high versus low tertile of total activity (OR = 0.36; 95% CI = 0.16–0.79). For postmenopausal women, vigorous physical activity and total activity (high versus low tertile) also had an inverse relationship with breast cancer (p<.05).

Data regarding the association of physical activity and breast cancer has been equivocal and lacking for African American women. This study found that modest levels of physical activity reduced breast cancer risk in this group. Targeted efforts are needed to encourage more African American women to engage in physical activity.

Breast cancer is the leading cancer diagnosed in US women. With more than 190,000 cases diagnosed each year, there are few prevention strategies. Known risk factors, such as genetic mutations and family history of cancer, account for only 30% of a woman’s risk for being diagnosed with breast cancer. Thus, lifestyle changes may reduce a woman’s breast cancer risk.

Research suggests that an increase in physical activity, in particular, among sedentary, post-menopausal women changes their hormone levels to reflect a lower risk of breast cancer diagnosis. Therefore, physical activity may be one of the best approaches towards the primary prevention of breast cancer.

The relationship between physical activity and breast risk is complex and may be explained by certain molecular mechanisms. It is plausible that physical activity could affect breast cancer risk because it also affects other risk factors, such as menstrual cycle, body mass, immune system, and hormones such as IGF. It is well accepted that physical inactivity increases one’s risk of obesity. Obesity has separately been considered a breast cancer risk factor and there is an inverse relationship between obesity and physical activity.

How physical activity and obesity might confound each other is unknown; obesity is positively associated with breast cancer risk in postmenopausal women but inversely associated with risk in premenopausal women. Further, some data suggests that adult weight gain, rather than BMI, affects breast cancer risk, especially among non users of hormonal replacement therapy.

Studies have shown a relationship between obesity and risk of breast cancer but in samples of mostly Caucasian women.Research, although equivocal, suggest that physical activity is inversely related to breast cancer risk in both pre- and postmenopausal women. Considering African American women have higher rates of being overweight or obese, limited physical activity may be a significant breast cancer risk factor in this population. Some data in African American women suggests that breast cancer risk is reduced with increased physical activity.

In a study by Adams-Campbell et al, strenuous physical activity was associated with a reduced risk of breast cancer in African American women. This study, however, focused only on strenuous physical activity levels. Bernstein et al also found that increased physical activity reduces breast cancer risk in African-American women. However, this study also lacked data on the effect of moderate intensity exercise in this population. More research is needed to determine whether moderate levels of physical activity can also reduce breast cancer risk in this population.

Cardio and Strength Training May Help You Live Longer, Study Says

A consensus is building among experts that both strength training and cardio‌ are important for longevity. Regular physical activity has many known health benefits, one of which is that it might help you live longer. But what’s still being determined are the types and duration of exercise that offer the most protection.

In a new study published in The British Journal of Sports Medicine, researchers found that while doing either aerobic exercise or strength training was associated with a lower risk of dying during the study’s time frame, regularly doing both — one to three hours a week of aerobic exercise and one to two weekly strength training sessions — was associated with an even lower mortality risk.

Switching from a sedentary lifestyle to a workout schedule is comparable to “smoking versus not smoking,” said Carver Coleman, a data scientist and one of the authors of the study.The paper is the latest evidence in a trend showing the importance of strength training in longevity and overall health.

“The study is exciting because it does support having a mix of both aerobic and strength training,” said Dr. Kenneth Koncilja, a gerontologist at the Cleveland Clinic, who was not involved in the study. “That is definitely something I talk with my patients about all the time.”

For the study, researchers used National Health Interview Survey data, which followed 416,420 American adults recruited between 1997 and 2014. Participants filled out questionnaires detailing the types of physical activity they had been doing, which included specifying how much moderate or vigorous exercise, along with how many sessions of muscle-strengthening exercises they did in a week.

After adjusting for factors such as age, gender, income, education, marital status and whether they had chronic conditions, such as diabetes, heart disease or cancer, researchers found that people who engaged in one hour of moderate to vigorous aerobic activity a week had a 15 percent lower mortality risk. Mortality risk was 27 percent lower for those who did three hours a week.

But those who also took part in one to two strength-training sessions per week had an even lower mortality risk — a full 40 percent lower than those who didn’t exercise at all. This was roughly the difference between a nonsmoker and someone with a half-a-pack-a-day habit.

Experts say it has been difficult to study longevity and strength training because so few people do it regularly. Even in the recent study, just 24 percent of participants did regular strength training (as opposed to 63 percent who said they did aerobic workouts). “Even with huge cohorts like we had here, the numbers are still relatively small,” said Arden Pope, an economist at Brigham Young University and one of the authors of the paper.

However, research is starting to catch up. In a recent meta-analysis, published February, also in The British Journal of Sports Medicine, researchers were able to quantify the effect of strength training on longevity outside of aerobic activity.

They found the largest reduction was associated with 30 to 60 minutes of strength training a week, with a 10 to 20 percent drop in the risk of mortality, cardiovascular disease and cancer. However, as Haruki Momma, a sports scientist at Tohoku University and one of the authors of the study, points out, there needs to be more research done to find the optimal amount of strength training.

Even though more research is needed, experts generally agree that regular strength training can have important benefits for healthy aging, including maintaining a high quality of life.

“You will function at a much higher level, for longer, if you have good muscle strength,” said Dr. Bruce Moseley, an orthopedic surgeon at Baylor College of Medicine.

Muscle strength is required for a number of daily activities, such as getting out of a chair, opening a jar of pickles, carrying groceries into the house or doing yardwork. However, “we progressively lose muscle mass as we age,” said Monica Ciolino, a physical therapist at Washington University at St. Louis.

This muscle loss usually starts in a person’s 30s and progresses with age. However, “we can absolutely fend off the negative effects” with regular strength training, Dr. Ciolino said. And it’s never too late to start. Research shows even septuagenarians with mobility issues can benefit from a regular strength-training program.

Dr. Moseley suggests aiming for a consistent strength-training schedule and easing into it to avoid overuse injuries.

“Keep it at a light and easy level at first,” he said. “Once your body starts getting adjusted, then you can start increasing.”

If you are still uncertain about certain exercises, he recommends seeking out expert advice through an exercise class or consulting with a personal trainer. The important thing, he said, is to get to started and to make it a habit. Not only can this help you live longer, it will improve your quality of life.

“When I ask people, ‘What does successful aging mean to you?’ people say they want to be independent, they want to maintain their function and quality of life, they want to do the things that they want to do,” Dr. Koncilja said. “It’s not necessarily just living as long as possible.”

Choose an activity and make sure it’s something you actually like or, if like is too strong of a word, at least feel comfortable doing, which will help keep you motivated.1 This can be anything that involves some kind of continuous, rhythmic movement that gets your heart rate up.

Ideas include home cardio exercises and workouts, walking, running, cycling, home workout videos or online fitness videos, cardio machines such as a treadmill, stationary bike, rowing machine, or elliptical trainer, exergames, organized or casual sports,Hate cardio? Anything that gets you moving can count: Walking around your house, dancing in your basement, strolling the mall, etc. Make it your own.

Choose the Days and Times You’ll Exercise

General guidelines suggest moderate cardio for 30-60 minutes most days of the week, but start with a) What you actually have time for and b) What you can actually handle. If you’re not sure, start with a basic program that’s 3-4 days a week.

Figure out how much time you’ll exercise. Again, this is based on how much time you actually have (not how much time you think you should have) and what you can handle. One reason we fail to stick to exercise is that we don’t work with our schedules as they actually are.2 If you really only have 10 minutes a day, then that’s what you use for your workouts.

Schedule and Prepare for Your Workouts

Put your workouts in your calendar just as you would any appointment. Treat it like something you would never miss such as a doctor’s appointment or a massage. Plan ahead and start to prepare for your workout well in advance. If you workout in the morning, gather your things the night before. If you like to workout in the evening, or after work, be sure to prep in the morning. You should have everything you need – Clothes, shoes, water, snack, heart rate monitor, phone, etc. ready and waiting before your workout. If it’s not, you’ll have one more reason to skip your workout.

Start Where You Are and Check-In Weekly

If you can’t do 30 minutes, do 5 or 10 or whatever you can do, and progress by adding a few minutes to each workout until you can go continuously for 30 minutes. Make notes of any difficulties you’re having and deal with them right away. If you’re finding it hard to fit in workouts, think of ways to do short bouts of exercise throughout the day.

Strive to work at a moderate intensity, in the low-middle end of your target heart rate zone. Don’t worry too much about working hard during the first few weeks, but do try to work at a level that feels like actual exercise.

Signs of Overtraining

Overtraining is a common problem with new exercisers.3 You may tend to do the amount of exercise you believe you need to lose weight or improve fitness and forget your body isn’t necessarily ready for that amount.

Pay attention to these warning signs of overdoing it such as loss of motivation to train, feeling more sore than usual, a higher resting heart rate, fatigue, sleep disturbances, and mood changes.

How to Prevent Overtraining

If you begin to experience signs of overtraining, back off of your workouts. At the very least, cut down on the time and/or intensity or give yourself a few days off completely.3 Backing off on frequency and intensity in a structured way is called a deload. Deloads are an important part of any workout program.

When you’re ready to return to your regular training, ease back into it, but keep things a little lighter than before. Pay attention to how your body feels before, during, and after your workouts. If you feel drained for the rest of the day, that may be a sign you need to lighten up on the intensity.

Another option when you are feeling overtrained is to try something different. Try yoga or just simple stretching as a way to relax, reduce the stress on your body and heal. Rest and recovery are key to success and this includes getting the proper amount of sleep and consuming enough calories to support your training.

Brain Drain: Scientists Look at Why Mental Exertion Triggers Exhaustion

It’s a familiar feeling on a Friday evening. After finishing a gruelling day’s work, you finally agree with friends on where to meet for a night out. But by the time you have figured out what to wear and where you left your keys, a night on the sofa begins to sound more appealing than one on the tiles.

Now, scientists think they may be able to explain why you feel so weary before you have even reached the bus stop: your brain has slowed down to manage the strain. The brain could suffer from something similar to the painful buildup of lactate in muscles during physical exercise. This could be why hard mental yards – and resisting the temptation to give up throughout the day – feel equally taxing.

Prolonged mental activity leads to the accumulation of a potentially toxic neurotransmitter in the prefrontal cortex, according to a study published in Current Biology. The researchers suggest the brain slows down its activity to manage the buildup, offering an explanation to why we feel tired.

“Even when you resist scratching an itch, for example, your brain is exerting cognitive control,” said Antonius Wiehler of the Paris Brain Institute, the first author of the study. Repeated demands on cognitive control functions can lead to fatigue, he said. The prefrontal cortex is the region of decision-making and cognitive control, which is applied when the brain overrides an impulse or fights any kind of temptation.

The team monitored the brain chemistry of 40 participants while they completed repetitive tasks on a computer. They formed two groups, who performed either hard tasks or easy tasks for over six hours. The researchers measured levels of a neurotransmitter in the prefrontal cortex. They found a greater accumulation of glutamate in participants who were given the harder tasks.

Work that involves a lot of thinking requires the brain to repeatedly resist the temptation to do something less demanding. Unsurprisingly, this can leave people feeling tired, but the brain chemistry behind it has remained unclear. Now, researchers suggest cognitive control may lead to the accumulation of glutamate in the brain – of which high levels can be harmful because it overexcites neural cells.

“We found that glutamate was accumulating in the region of the brain which controls the tasks we set participants,” said Wiehler. “Our understanding is that the brain has some kind of clearance mechanism to counteract this, which may slow down activity.” The researchers posit that mental fatigue could be linked to recycling the glutamate that builds up during neural activity. “The accumulated glutamate needs to be cleared away, which we think is likely happening during sleep,” said Wiehler.

When participants were asked to report their level of fatigue, no definitive link between glutamate and fatigue was found – the groups performing hard and easy tasks recorded the same tiredness. Researchers said this could be due to fatigue being subjective, and those doing the easy task were unaware of the difficulty of the other.

“The fact that glutamate levels don’t track the reported fatigue is slightly disappointing, but not surprising because there is often a dissociation between biological features and self-reported fatigue,” said Dr Anna Kuppuswamy from the Institute of Neurology at University College London, who was not involved in the study.

The researchers monitored only glutamate but suggest other related substances could be linked to fatigue. “The study measures a single neurotransmitter in a very specific part of the brain, so we have to look at it more globally,” said Kuppuswamy.

But the results were encouraging, she added. “We know that during physical exercise lactate accumulates in the muscles, leading to muscle fatigue. It is kind of intuitive that something similar happens in the brain and this is good first evidence to suggest that.”

Mental exhaustion is a feeling of extreme tiredness, characterized by other feelings including apathy, cynicism, and irritability. You may be mentally exhausted if you’ve recently undergone long-term stress, find it hard to focus on tasks or lack interest in activities you usually enjoy.

Mental exhaustion often happens as a result of overuse, like physical overuse injuries. Even though you can’t point to it, it has more in common with repetitive stress injuries, like carpal tunnel or tennis elbow. Rather than overstressing a muscle group, mental and emotional exhaustion come from overstressing your mind.

Mental exhaustion is completely possible and is probably more common than it should be. After a long period of stress or time of intense emotions, mental exhaustion is bound to happen. Just like our bodies show symptoms after we push too hard, our minds are bound to display signs of mental exhaustion if we don’t take proper care.

Mental exhaustion can be caused by many things. Typically, though, people feel mentally tired after experiencing long-term stress. This is especially true if the stressors increase a person’s cognitive load or reduce their resources. 

For example, you may be responsible for completing a challenging project with many moving parts and tradeoffs. This would require a high level of project management skills and political savvy (e.g., increased load). 

Another example would be traveling for work. Constantly changing time zones would leave you feeling jet-lagged (reduced resources). Many stressors involve both reduced resources and increased load. 

Work travel to an unfamiliar country where you don’t speak the language amps up the cognitive load. Taking care of a sick family member may involve coordinating medical care and interpreting unfamiliar terms while managing emotions (increased load). But you may also be getting less sleep (reduced resources). Over time, increased responsibility and stress plus poor self-care can result in mental exhaustion.

Though a wide variety of stressors can cause you to feel mentally drained, we’ve boiled down the 7 most common causes of mental exhaustion below. 

1. Chronic stress

This is the most frequent cause of mental exhaustion. Chronic stress keeps your brain — and body — on high alert all the time. Over time, this begins to wear away at your well-being. Chronic stress can also lead to empathy or compassion fatigue. It can become difficult to muster an emotional response to the constant strain.

2. Uncertainty

The human stress response was designed to work efficiently in the face of short-term stress (think fight-or-flight). However, it’s a much less effective response to a constant, nagging feeling of uncertainty. Unfortunately, uncertainty has become far too normal of a feeling since the start of COVID-19 pandemic. This has made mental exhaustion more common than ever. 

3. Work stress

Stress at work can take many forms. It can arise from a values mismatch, difficulty managing tasks and priorities, or a high-demand, risk-oriented job. Some jobs (or programs of study) involve a lot of new learning. They could also require processing and making sense of a lot of information. Whatever the reason, it’s not always possible to leave work at work. Left unchecked, workplace stress can even evolve into burnout. Your work stress could bleed into your weekends and ultimately, develop into a bad case of the Sunday scaries

4. Family issues

Few things are more stressful than worrying about a family member. Being a caregiver for young children, sick relatives, or aging parents can be mentally taxing. Even if everyone’s healthy, families can bring all kinds of stressors. Divorce, disagreements, and estrangements have a way of following you into all areas of your life. Ultimately, family troubles can be a big cause of mental exhaustion.

5. Juggling multiple commitments

In addition to caring for family, many people have other commitments on their plate — those commitments come with details, schedules, logistics, and challenges. Balancing an intensive school or training program, a second job, or a freelance business can leave you feeling like you’re never “off.” If you’re not able to, or don’t know how to, manage your priorities, you’re at risk of becoming mentally drained.

6. Emotional stress

There are dozens of things that can cause emotional stress. No matter the cause, the experience is similar. Constant negative feelings, events, and circumstances can make it difficult to relax. This emotional exhaustion can quickly lead to mental fatigue.

7. Poor self-care

Without gas in the tank, you won’t get very far. When you get busy or you’re feeling a bit down, it’s easy to neglect self-care. However, over time this will affect your ability to be resilient in the face of stressful situations. If you have a chronic illness, like multiple sclerosis or chronic fatigue syndrome, brain fog may be a side effect. Rather than pushing through, it’s important to be especially diligent about self-care.

How To Avoid The Most Common Workout Injuries, According To Experts

A good rule of thumb: Don’t increase your workout volume more than 10% per week. And every four to eight weeks, give your body a rest by significantly reducing the volume and intensity of your workouts. “This ‘three steps forward, one step back’ approach requires discipline and isn’t always fun,” Matthews said, “but it’s the best way to make your body more resilient and durable.”Cross-training is another good idea, which the American Academy of Orthopaedic Surgeons endorses. Since your body’s soft tissues are working in different ways or even resting when you bike versus swim or play tennis, it’s an easy preventive measure.

Diet, stress, sleep may increase risk, too

Avoiding soft tissue injuries isn’t necessarily all about training, however. Research suggests major changes in your environment may affect your risk of injury, too, Cheatham said, such as poor nutrition, stress and lack of sleep. Cheatham is also a member of the National Academy of Sports Medicine Scientific Advisory Board. If you get less than seven hours of sleep at night for more than two weeks, your risk of musculoskeletal injury rises 1.7 times, found a 2021 study published in the journal Current Sports Medicine Reports. So eat well, get plenty of sleep and perhaps skip tough workouts when your stress levels are high.

And what about stretching? Stretching, warm-ups, a post-workout meal and other practices have long been touted to help avoid injury, yet there is no evidence to support these moves, Matthews said. Still, developing a strong core is always helpful, said physical therapist Aime Maranan, owner of Skillz Physical Therapy in Evanston, Illinois.”If the muscles in your core aren’t strong enough to withstand hours of training, their strength will go down, then the stability of the spine will go down, and then your nerves and soft tissues will be irritated,” she said. “It’s a domino effect.”

Core exercises such as the plank are good, she said, or holding the tabletop position, where you lie on your back with your hips and knees at a 90-degree angle. The quadruped is valuable, too. This exercise involves getting onto your hands and knees, contracting your core, then alternating an extension of your right arm and left leg with an extension of your left arm and right leg. Yet these exercises have to be performed properly, or ironically, they could cause a soft issue injury. So consult with a professional before doing them on your own to ensure proper form. This could be your physical therapist, chiropractor, personal trainer or fitness instructor.

Take any injuries seriously

If you do get injured despite your best precautions, take it seriously. “Even when people realize they have a soft tissue injury, they often carry on with their program and whistle past the graveyard, hoping it gets better with time,” Matthews said. “More often than not, it just gets worse and worse until it hurts badly enough that the person simply can’t train due to the pain.”Instead of ignoring that muscle or ligament strain, see a qualified health care provider and expect to spend a few weeks to a month or more recovering, depending on the severity of the injury, your age and other factors. Most importantly, complete your entire rehabilitation process so another injury doesn’t occur, Cheatham said. No stopping the minute you start feeling a little better.

A positive mindset is also key to a speedy recovery. “If you think you will not get any better, you will not get any better. If you think you will get injured again, you will get injured again,” Maranan said. “It starts with your mindset, then religiously doing your home exercises and your post-exercise recovery routine.” And remember, stay mindful to stay true to form.

Nearly two million people every year suffer sports-related injuries and receive treatment in emergency departments. Fatigue is a large contributing factor that results in many sport injuries. There are times where an athlete may participate on low energy leading to the deterioration in technique or form, resulting in a slower reaction time, loss in stability of muscle joints, and allowing an injury to occur.

For both sexes the most common areas injured are the knee and ankle, with sprains/strains being the most common areas for injury. Injuries involving the patellofemoral articulation are significantly more frequent among females. The sport with the highest injury rate in the United States is American football, with greater than 12 times the number of injuries seen in the next most common sport.

Soft tissue injuries

When soft tissue experiences trauma the dead and damaged cells release chemicals, which initiate an inflammatory response. The small blood vessels that are damaged become dilated which produce bleeding within the tissue. The body’s normal response includes forming a small blood clot in order to stop the bleeding and allows a clot of special cells, called fibroblasts, to form. This begins the healing process by laying down scar tissue.

Therefore, the inflammatory stage is the first phase of healing. However, too much of an inflammatory response in the early stage can indicate that the healing process takes longer and a return to activity is delayed. Sports injury treatments are intended to minimize the inflammatory phase of an injury, so that the overall healing process is accelerated. Intrinsic and extrinsic factors are determinant for the healing process.

Soft tissue injuries can be generally grouped into three categories: contusions, abrasions and lacerations. Contusions or bruises are the simplest and most common soft tissue injury and is usually a result of blunt force trauma. Severe contusions may involve deeper structures and can include nerve or vascular injury. Abrasions are superficial injuries to the skin no deeper than the epidermis tissue layer, and bleeding, if present, is minimal.

Minor abrasions generally do not scar, but deeper abrasions generally bleed and may scar. Lastly, sports-related lacerations are caused by blunt trauma and result in burst-type open wounds, often with jagged irregular edges. Facial lacerations are the most variable of the soft tissue injuries that athletes can sustain. They can occur intraorally and extraorally, vary from a superficial skin nick to a through and through lip laceration, or involve significant vascular disruption or injury to collateral vital structures.

Hard tissue injuries

Types of hard tissue injuries can include dental and bone injuries and are less frequent than soft tissue injuries in sport, but are often more serious.Hard tissue injuries to teeth and bones can occur with contusions, such as Battle sign, which indicates basilar skull fracture, and so-called raccoon eyes, which indicate mid-face fractures. However, tooth fractures are the most common type of tooth injury, and can be categorized as crown infractions, enamel-only fracture, enamel-dentin fractures, and fractures that extend through the enamel and dentin into the pulp which are defined below.

  • Crown infractions are characterized by a disruption of the enamel prisms from a traumatic force, these injuries typically present as small cracks that affect only the enamel.
  • Enamel-only fractures are mild and often appear as roughness along the edge of the tooth crown. These injuries typically can go unnoticed by the athlete as they are usually not sensitive to the touch or to temperature changes. Enamel-only fractures are not considered dental emergencies and immediate care is not needed.
  • Enamel-Dentin crown fractures typically present as a tooth fracture confined to enamel and dentin with loss of tooth structure, but not exposing the pulp. The athlete often will report sensitivity to air, cold or touch, but the athlete can return to play as tolerated and referral can be delayed up to 24 hours.
  • Enamel-Dentin-Pulp fractures extend through the enamel and dentin and into the pulp. If the pulp is vital, a focal spot of hemorrhage will be noticeable within the yellow dentin layer and the athlete may report acute pain. Referral to a trauma-ready dentist should occur as soon as possible.

In addition to tooth fractures, there are several types of bone fractures as well. These types being closed or simple, open or compound, greenstick, hairline, complicated, comminuted, avulsion, and compression. A complicated fracture is when the structures surrounding the fracture are injured, such as blood vessels, organs, nerves, etc.

Overuse injuries

Overuse injuries can be defined as injuries that result from a mechanism of repetitive and cumulative micro-trauma, without a specific onset incident.Rapid changes in physical growth can make children vulnerable to overuse injuries, and these types of injuries are increasing among youth populations. Overuse injuries can usually be classified into 4 types/stages, these include:

  • Pain in the affected area during activity (which does not affect performance)
  • Pain in the affected area during activity (which does restrict performance)
  • Pain in the affected area after activity
  • Chronic pain in the affected area, even after resting

Predictive Indicators of Overuse Injuries in Adolescent Endurance Athletes, runners seem to account for the majority of injuries (up to 80 percent) with the majority of these injuries (more than two-thirds), occurring in the lower extremity and being of an overuse nature.Although incidence rates in senior athletics has been reported as 3.9 injuries/1000 hours of practice, specific injury incidence in youth track and field varies among disciplines; whereas an overall incidence of 0.89 injuries/1000 hours has been reported for high school track and field athletes. In addition, long-distance runners have showed a 19 times higher incidence (17 injuries/1000 hours) than other disciplines.

Head and neck injuries

Head and neck injuries can include a variety of pathologies from sprains, strains and fractures to traumatic brain injuries and spinal cord injuries. Sprains and strains can occur from an abrupt rotation or whipping motion, such as whiplash.Stress injuries (stress fractures and stress reactions) of the lumbosacral region are one of the causes of sports-related lower back pain in young individuals. The onset of the observed cervical fractures in sports injury were likely due to continued momentum that transferred loads superiorly through the neck, which likely exacerbated the injuries to the occipital condyles and the upper cervical vertebrae.

Researchers have reported that 3-25% of cervical spine injuries actually occur after the initial traumatic event and are caused or exacerbated by improper handling during early stages of management or patient transport.One of the more common head or neck injuries that occurs in sports is a concussion. A concussion is a type of mild traumatic brain injury resulting in a chemical change in the brain and has potential to cause damage to brain tissue.

This can occur when a person sustains a hit or blow that cause the head and brain to move quickly, causing the brain to bounce in the skull. According to an epidemiological study published in the Journal of Athletic Training, the incidence of concussions from 27 high school sports was 3.89 sports-related concussions per 10,000 athlete exposures.
